【摘要】：目的探討OSAHS患者認(rèn)知功能和言語識(shí)別能力的關(guān)系。方法選擇青年男性O(shè)SAHS患者60例,年齡18~40歲,根據(jù)臨床診斷標(biāo)準(zhǔn)及多導(dǎo)睡眠圖(polysomnogram,PSG)結(jié)果將患者按照呼吸暫停低通氣指數(shù)(AHI)及低氧血癥程度分為:輕度組19例,中度組20例,重度組21例;對(duì)照組為正常青年男性(年齡18~40歲)25例。對(duì)各組首先進(jìn)行蒙特利爾認(rèn)知評(píng)估量表(Montreal cognitive assessment,MoCA)測(cè)試,然后應(yīng)用普通話言語測(cè)聽材料(MSTMs)中的15張句表(每表50個(gè)關(guān)鍵詞)進(jìn)行言語識(shí)別測(cè)試。結(jié)果 1對(duì)照組和輕、中、重度OSAHS患者組MoCA得分總分分別為:28.52±1.16、27.32±1.16、25.85±1.23、24.52±1.69分,四組之間兩兩比較差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。2刺激聲強(qiáng)度為22、24、26dB SPL時(shí),重度OSAHS患者的言語識(shí)別率(%)分別為:35.4±22.6、56.3±23.9、75.2±16.5,較輕度組(38.4±23.5、58.3±25.5、79.2±18.5)、中度組(38.8±21.6、58.7±22.7、78.5±16.7)、對(duì)照組(39.4±23.5、60.3±24.3、80.2±16.4)均降低(P均0.05)。3各組50%識(shí)別率所需的給聲強(qiáng)度分別為:輕度組3.66±0.50dB SPL、中度組3.72±0.55dB SPL、重度組4.15±0.80dB SPL、對(duì)照組3.62±0.41dB SPL,重度組與其余三組比較差異有統(tǒng)計(jì)學(xué)意義(P0.01)。結(jié)論 OSAHS患者隨著缺氧和病情程度加重,言語識(shí)別能力呈下降趨勢(shì),這可能是認(rèn)知評(píng)估量表得分降低的重要相關(guān)因素。
[Abstract]:Objective to investigate the relationship between cognitive function and speech recognition in patients with OSAHS. Methods 60 young male patients with OSAHS, aged 1840 years, were divided into mild group (19 cases) and moderate group (20 cases) according to apnea hypopnea index (AHIH) and degree of hypoxemia according to the clinical diagnostic criteria and polysomnogrammetry (PSG) results. There were 21 cases in severe group and 25 cases in normal young male (18 ~ 40 years old). First, the Montreal cognitive assessment test was performed in each group, and then 15 sentence tables (50 keywords per table) were used in the speech audiometry of Putonghua. Results 1 the total score of MoCA in the control group and the mild, moderate and severe OSAHS group was 28. 52 鹵1. 1610. 32 鹵1. 1625. 85 鹵1. 23 and 24. 52 鹵1. 69, respectively.
