【摘要】：目的尋找替代自體皮膚移植矯正義眼臺植入術后結膜囊畸形的材料,避免供皮區(qū)的創(chuàng)傷,減少結膜囊攣縮。方法我科2012年6月~2016年6月收治已植入義眼臺結膜囊畸形患者12例,包括3例義眼臺暴露并結膜囊狹窄患者;在狹窄結膜囊中央切開結膜及淺層筋膜,于淺層筋膜下分離結膜及淺層筋膜與義眼臺表面筋膜組織粘連,分離至眶緣。制作大小合適的心臟滌綸補片,覆蓋于結膜囊中央結膜缺損暴露區(qū),補片周邊固定于眶緣骨膜,結膜及淺層筋膜邊緣固定于穹窿底部心臟滌綸補片上,結膜囊內植入隔膜,永久瞼緣粘連。結果 8例術后3月分離瞼緣粘連后植入義眼,2例行再次下穹窿成形術后植入義眼,外觀均得到明顯改善;2例心臟滌綸補片排斥并感染去除后應用自體全厚皮片行全結膜囊再造成功。結論心臟滌綸補片是修復義眼臺植入后無眼球結膜囊畸形及義眼臺暴露的理想材料,但不宜用于重度結膜囊狹窄或結膜囊閉鎖患者。
[Abstract]:Objective to find a substitute for autologous skin graft for correction of conjunctival sac deformity after orthodontic implant surgery in order to avoid the trauma of the skin donor area and reduce the contracture of conjunctival sac. Methods 12 patients with conjunctival sac deformity implanted in the prosthesis were treated in June June 2012 ~2016, including 3 cases of prosthetic eye exposure and conjunctival sac stenosis, and central incision in the narrow conjunctival sac. The conjunctiva and superficial fascia were separated from the superficial fascia and the superficial fascia and the surface fascia of the prosthetic eye, and separated to the orbital margin. The proper size of the heart Dacron patch was made to cover the central conjunctival defect exposure area of the conjunctival sac. The patch was fixed to the periosteum of the orbital margin, and the conjunctiva and the superficial fascia were fixed to the heart of the fornix. On the Dacron patch, the septum was implanted in the conjunctival sac and permanent conglutination was implanted in the conjunctival sac. Results 8 cases were implanted into the prosthesis after the separation of the palpebral margin in March. 2 cases were implanted with the prosthetic eye after the reconstruction of the lower fornix, and the appearance was obviously improved. 2 cases of heart Dacron patch repulsion and infection were removed and the whole conjunctival sac was successful. Conclusion the heart is successful. The Dacron patch is an ideal material for repairing the deformity of the conjunctival sac and the exposure of the prosthesis, but it is not suitable for the patients with severe conjunctival sac stenosis or conjunctival atresia.
