【摘要】：目的:研究變應(yīng)性鼻炎伴腺樣體肥大患兒應(yīng)用丙酸氟替卡松噴劑治療的有效性和安全性。方法:選擇變應(yīng)性鼻炎伴腺樣體生長(zhǎng)的患兒160例做為觀察組,其中腺樣體肥大者109例;選擇同期單純腺樣體生長(zhǎng)的患兒157例做為對(duì)照組,其中腺樣體肥大者114例。兩組患兒均接受丙酸氟替卡松噴劑治療3個(gè)月。觀察比較兩組患兒治療前后鼻鏡下腺樣體體積、癥狀評(píng)分變化,以及藥品不良反應(yīng)發(fā)生情況。結(jié)果:變應(yīng)性鼻炎伴隨腺樣體肥大患兒多發(fā)于3~6歲。治療后,觀察組有效腺樣體縮小患者88例(80.73%),顯著多于對(duì)照組的67例(58.77%)(P0.01);對(duì)照組治療后腺樣體肥大的患者為55例(35.03%),明顯多于觀察組的44例(68.13%,P0.05)。治療前后觀察組患者的鼻塞癥狀積分差值、打鼾癥狀積分差值、張口呼吸癥狀積分差值顯著大于對(duì)照組(P0.05)。結(jié)論:丙酸氟替卡松噴劑對(duì)于縮小變應(yīng)性鼻炎伴腺樣體肥大患兒腺樣體體積比單純腺樣體肥大患兒更明顯,且能更顯著改善打鼾、鼻塞、張口呼吸癥狀。
[Abstract]:Objective: To study the efficacy and safety of the use of fluticasone propionate spray in children with allergic rhinitis and adenoid hypertrophy. Methods: 160 children with allergic rhinitis with adenoid growth were selected as observation group, of which 109 cases were adenoid hypertrophy, and 157 children with adenoid growth were selected as control group, in which adenoid samples were selected. There were 114 cases of hypertrophy of body. The two groups were treated with fluticasone propionate for 3 months. The volume of adenoid body under the nasal mirror, the symptom score change, and the adverse drug reaction were observed and compared before and after treatment in the two groups. Results: the allergic rhinitis accompanied with adenoid hypertrophy was 3~6 years old. After treatment, the observation group was effective adenoid contraction. 88 cases of small patients (80.73%) were significantly more than 67 cases (58.77%) in the control group (P0.01), and 55 cases (35.03%) of adenoid hypertrophy in the control group were more than 44 cases (68.13%, P0.05) in the observation group. The difference value of nasal congestion symptom score, the integral difference of snoring symptoms and the score difference of mouth breathing symptoms were significantly greater than those of the control group before and after treatment. Group (P0.05). Conclusion: the fluticasone propionate spray is more obvious in reducing the volume of adenoid hypertrophy in children with allergic rhinitis and adenoid hypertrophy, and can improve snoring, nasal congestion and mouth breathing.
