【摘要】：目的探討商業(yè)化軟件計(jì)算球后脂肪體積,分析其與甲狀腺相關(guān)性眼病(TAO)進(jìn)展及預(yù)后的關(guān)系。方法收集2016年1月~2016年12月在我院內(nèi)分泌科確診的35例(70個(gè)眼眶)TAO患者的臨床資料。測量1.5T眼眶MRI球后脂肪體積及眼外肌信號強(qiáng)度比值(SIR)分析其與臨床各項(xiàng)指標(biāo)的相關(guān)性,并收集12例(24個(gè)眼眶)健康人測量球后脂肪體積,初步比較TAO組及健康組體積的差異。結(jié)果脂肪體積與病程成正相關(guān)(r=0.480,P0.01),病程6個(gè)月以內(nèi)組與6~12個(gè)月組相比,脂肪體積差異不顯著(P=0.084)。病程6個(gè)月以內(nèi)組及病程6~12個(gè)月組球后脂肪體積均顯著低于病程大于12個(gè)月組(P0.01,P0.05)。脂肪體積與突眼度存在相關(guān)性(r=0.622,P0.01),突眼度每增加1 mm,球后脂肪體積將增加0.88 m L。臨床活動(dòng)性評分(CAS)與SIR值及促甲狀腺素受體抗體(TRAb)存在相關(guān)性(r=0.536,r=0.416,P0.01)。TAO組球后脂肪體積顯著高于正常組(P0.01)。結(jié)論 TAO病程1年以上可能是球后脂肪組織增多的高峰階段,球后脂肪體積結(jié)合SIR值的測量有助于最佳激素治療時(shí)機(jī)的探索及預(yù)后分析。
[Abstract]:Objective to investigate the relationship between retrobulbar fat volume and the progression and prognosis of thyroid associated ophthalmopathy (TAO) by commercial software. Methods from January 2016 to December 2016, the clinical data of 35 cases (70 cases of orbital TAO) diagnosed in the department of endocrine department in our hospital were collected. Retrobulbar fat volume and signal intensity ratio of extraocular muscle (EOMG) were measured at 1.5T. The correlation between the volume of fat and clinical indexes was analyzed. Twelve healthy subjects (24 eyes) were collected to measure the volume of retrobulbar fat, and the difference between TAO group and healthy group was preliminarily compared. Results there was a positive correlation between the fat volume and the course of the disease. There was no significant difference in fat volume between the 6 month group and the 6 ~ 12 month group. The volume of retroglobular fat in 6 months group and 6 ~ 12 month group was significantly lower than that in longer than 12 month group. There was a correlation between the volume of fat and the degree of exophthalmos. The volume of fat behind the ball increased by 0.88 mL with the increase of 1 mm in the degree of exophthalmos. There was a significant correlation between clinical activity score and SIR and thyrotropin receptor antibody (TRAb). The volume of retroglobular fat in the TAO group was significantly higher than that in the normal group (P 0.01). Conclusion the course of TAO for more than one year may be the peak stage for the increase of retrobulbar adipose tissue. The measurement of retrobulbar fat volume combined with SIR value may be helpful to explore the best time of hormone therapy and to analyze the prognosis.
