【摘要】：目的探討鼓室內注射利多卡因聯(lián)合掩蔽及心理輔導綜合治療特發(fā)性耳鳴的臨床療效。方法選擇符合納入標準的耳鳴患者177例,隨機分為鼓室內注射利多卡因聯(lián)合掩蔽組(利多卡因組)59例、心理輔導聯(lián)合掩蔽組(心理輔導組)59例、鼓室內注射利多卡因聯(lián)合心理輔導及掩蔽組(三聯(lián)組)59例,分別在治療前、治療后1月、治療后3月,通過耳鳴嚴重程度評估指標及評分標準,及SCL-90評分,評估三種治療方式的療效及穩(wěn)定性。結果 1治療后1月三組間療效比較,Hc=7.94,P=0.019,治療后3月三組間療效比較Hc=7.01,P=0.03,差異均顯著,兩兩比較結果顯示,三聯(lián)組與其他兩組間差異顯著。2三組患者治療后SCL-90評分在軀體化、人際敏感、抑郁、焦慮、敵對、偏執(zhí)因子方面差異顯著(P0.05),心理輔導組治療前后SCL-90評分在軀體化、人際敏感、抑郁、焦慮、偏執(zhí)因子方面差異顯著(P0.05),三聯(lián)組治療前后SCL-90評分在軀體化、人際敏感、抑郁、焦慮、敵對、偏執(zhí)因子方面差異顯著(P0.05),利多卡因組在治療前后各因子間無顯著差異。結論三聯(lián)組治療耳鳴療效優(yōu)于其他兩組,三種治療方法均有一定的穩(wěn)定性,采用心理干預措施能對耳鳴患者具有較好的療效。
[Abstract]:Objective to investigate the clinical effect of intratympanic injection of lidocaine combined with masking and psychological counseling in the treatment of idiopathic tinnitus.Methods 177 patients with tinnitus who met the inclusion criteria were randomly divided into two groups: intratympanic injection of lidocaine and masking group (59 cases in lidocaine group and 59 cases in psychological counseling group).Intratympanic injection of lidocaine in combination with psychological counseling and shelter group (59 cases in the three groups) were evaluated by the assessment index and scoring standard of tinnitus severity and SCL-90 score before treatment, 1 month after treatment and 3 months after treatment, respectively.To evaluate the efficacy and stability of the three treatments.Results (1) the comparison of the three groups' curative effect at 1 month after treatment was compared with that of the three groups after treatment. The results showed that the SCL-90 score of the three groups was somatization after treatment, and the difference between the three groups was significant (P < 0.01), and the difference between the three groups and the other two groups was significant (P < 0.01), and the difference between the three groups was significant. 2. After treatment, the SCL-90 score of the three groups was somatization.The factors of interpersonal sensitivity, depression, anxiety, hostility and paranoid ideation were significantly different (P 0.05). The SCL-90 scores of psychological counseling group were somatization, interpersonal sensitivity, depression, anxiety before and after treatment.There was significant difference in paranoid ideation factor (P0.05), SCL-90 score before and after treatment in the triple group (P 0.05). There was no significant difference in the scores of somatization, interpersonal sensitivity, depression, anxiety, hostility and paranoid ideation (P 0.05), but there was no significant difference between the factors before and after treatment in lidocaine group.Conclusion the treatment of tinnitus in the triple group is better than that in the other two groups, and the three treatment methods have certain stability. Psychological intervention can be used to treat tinnitus patients with better curative effect.
