【摘要】：目的觀察并對(duì)比小瞳孔白內(nèi)障手術(shù)中虹膜拉鉤與放射狀瞳孔緣虹膜切開擴(kuò)瞳的并發(fā)癥及視力預(yù)后。設(shè)計(jì)回顧性比較性病例系列。研究對(duì)象2012年4月至2015年6月在廣州市中西醫(yī)結(jié)合醫(yī)院接受超聲乳化白內(nèi)障吸出聯(lián)合人工晶狀體植入術(shù)的83例(90眼)小瞳孔白內(nèi)障患者。方法 50眼行虹膜拉鉤擴(kuò)瞳(研究組),40眼行放射狀瞳孔緣虹膜切開擴(kuò)瞳(對(duì)照組),記錄兩組的圍手術(shù)期指標(biāo)、術(shù)后并發(fā)癥及視力恢復(fù)情況。主要指標(biāo)超聲乳化時(shí)間、手術(shù)時(shí)間、術(shù)中術(shù)后并發(fā)癥及術(shù)后視力。結(jié)果對(duì)照組平均超聲乳化時(shí)間顯著高于研究組(104.8±17.9 s和57.8±11.9 s)(t=12.835,P0.001);對(duì)照組平均手術(shù)總時(shí)間明顯低于研究組(20.1±3.8 min和29.2±5.8 min)(t=-6.468,P0.001);對(duì)照組術(shù)中虹膜損傷及出血顯著高于研究組(P0.001);對(duì)照組術(shù)后1天矯正視力≥0.3者(40%),低于研究組(72%)(χ~2=9.328,P=0.002),對(duì)照組術(shù)后3個(gè)月矯正視力≥0.3者(80%),低于研究組(90%)(χ~2=1.798,P=0.180);研究組術(shù)后1天瞳孔變形率顯著低于對(duì)照組(68%和95%)(χ~2=10.125,P=0.001);研究組術(shù)后1周瞳孔變形率顯著低于對(duì)照組(20%和90%)(χ~2=46.557,P0.001)。研究組和對(duì)照組的角膜水腫率分別為86%和90%(χ~2=0.331,P=0.565),但兩組的角膜水腫程度比較,差異有統(tǒng)計(jì)學(xué)意義(Z=11.318,P=0.003);研究組前房反應(yīng)率68%,顯著低于對(duì)照組的95%(χ~2=10.125,P=0.001)。結(jié)論小瞳孔白內(nèi)障手術(shù)中虹膜拉鉤擴(kuò)瞳,具有安全系數(shù)高、不易出血和引起炎癥的優(yōu)點(diǎn),對(duì)術(shù)后瞳孔恢復(fù)形態(tài)和功能有積極意義。
[Abstract]:Objective to observe and compare the complications and visual prognosis of iris retractor and radial pupil edge iridotomy in small pupillary cataract surgery.A retrospective comparative case series was designed.Participants from April 2012 to June 2015, 83 patients (90 eyes) with small pupillary cataract underwent phacoemulsification combined with intraocular lens implantation in Guangzhou Integrated Chinese and Western Medicine Hospital.Methods 50 eyes were treated with iris retractor dilatation (40 eyes of study group) underwent iris dilatation (control group). The perioperative indexes, postoperative complications and visual acuity were recorded in both groups.Main outcome measures phacoemulsification time, operative time, postoperative complications and postoperative visual acuity.Results the average phacoemulsification time in the control group was significantly higher than that in the study group (104.8 鹵17.9 s and 57.8 鹵11.9 s), and the mean total operation time in the control group was significantly lower than that in the study group (20.1 鹵3.8 min and 29.2 鹵5.8 mint-6.468 min, P 0.001). The iris injury and bleeding in the control group were significantly higher than that in the study group (P 0.001).The pore deformation rate was significantly lower than that in the control group (20% and 90% respectively).The corneal edema rates of the study group and the control group were 86% and 90% respectively (蠂 ~ (2) 0.331P ~ (0.565)), but there was a significant difference in the degree of corneal edema between the two groups. The response rate of the anterior chamber in the study group was significantly lower than that in the control group (蠂 ~ (2 10.125) P ~ (0.001)) and the response rate of the anterior chamber in the study group was significantly lower than that in the control group (蠂 ~ (210.125) P ~ (0.001) P ~ (0.001)).Conclusion the dilated pupil of iris retractor in small pupil cataract surgery has the advantages of high safety factor, not easy bleeding and inflammation, which has positive significance for the recovery of pupil shape and function.
