【摘要】：目的:對(duì)鼓室內(nèi)激素初始治療突發(fā)性聾的Meta分析或系統(tǒng)評(píng)價(jià)進(jìn)行再評(píng)價(jià)研究。方法:計(jì)算機(jī)檢索國內(nèi)外相關(guān)數(shù)據(jù)庫,收集鼓室內(nèi)激素初始治療突發(fā)性聾的Meta分析或系統(tǒng)評(píng)價(jià),按照納入、排除標(biāo)準(zhǔn)篩選文獻(xiàn),提取出重要數(shù)據(jù)。采用AMSTAR法對(duì)其進(jìn)行方法學(xué)質(zhì)量評(píng)價(jià),應(yīng)用GRADE評(píng)估工具對(duì)文獻(xiàn)的主要結(jié)局指標(biāo)進(jìn)行證據(jù)質(zhì)量分級(jí)。結(jié)果:共納入16個(gè)相關(guān)的系統(tǒng)評(píng)價(jià)或Meta分析,其中中文6篇、英文10篇,文獻(xiàn)發(fā)表時(shí)間2009-2016年;AMSTAR評(píng)分為4~9分,14個(gè)研究為中等質(zhì)量,2個(gè)研究為高質(zhì)量。鼓室內(nèi)激素初始治療突發(fā)性聾的療效不肯定。主要為兩種結(jié)論:一是認(rèn)為其療效與全身激素治療相當(dāng);二是認(rèn)為鼓室內(nèi)激素初始治療突發(fā)性聾的療效優(yōu)于全身應(yīng)用,部分學(xué)者推薦作為一線治療。結(jié)論:當(dāng)前鼓室內(nèi)激素初始治療突發(fā)性聾的系統(tǒng)評(píng)價(jià)的方法學(xué)質(zhì)量水平多處于中等,結(jié)論的證據(jù)水平不高,對(duì)患者其他重要的結(jié)局指標(biāo)的關(guān)注度不足。因此,更多的高質(zhì)量的研究是我們的目標(biāo)。
[Abstract]:Objective: to study the Meta analysis or systematic evaluation of the initial treatment of sudden deafness with intratympanic hormone.Methods: relevant databases at home and abroad were searched and Meta analysis or systematic evaluation were collected for the initial treatment of sudden deafness by hormone in tympanic chamber. The important data were extracted according to the inclusion and exclusion criteria.The AMSTAR method was used to evaluate the methodological quality and the GRADE evaluation tool was used to classify the main outcome indicators of the literature.Results: a total of 16 related systematic evaluations or Meta analyses were included, including 6 in Chinese and 10 in English. The time of publication was 4 ~ 9 in 2009-2016, 14 were of medium quality and 2 were of high quality.The effect of intracympanic hormone on sudden deafness is uncertain.There are two main conclusions: one is that the curative effect is equivalent to the whole body hormone treatment, the other is that the initial treatment of sudden deafness by hormone in tympanic chamber is better than that in the whole body, some scholars recommend it as a first line treatment.Conclusion: at present, the systematic evaluation of the initial treatment of sudden deafness by steroid in tympanic chamber is mostly of moderate quality, but the level of evidence is not high, and the attention to other important outcome indexes of patients is not enough.Therefore, more high-quality research is our goal.
