【摘要】：目的探討鼻腔鼻咽部淋巴瘤的臨床特點、病理分型及類型構(gòu)成。方法回顧性分析首都醫(yī)科大學附屬北京友誼醫(yī)院病理科319例鼻腔鼻咽部淋巴瘤會診病例的臨床資料、病理組織學特征及免疫表型,按WHO(2008)分類標準進行病理診斷及分類,并對統(tǒng)計結(jié)果進行對比分析。結(jié)果 319例會診病例中,3例(0.9%)診斷為霍奇金淋巴瘤混合細胞型,考慮腫瘤累及鼻咽,建議全身檢查;316例均為非霍奇金淋巴瘤(99.1%),包含非霍奇金T和NK細胞起源淋巴瘤177例(56.0%)、B細胞起源淋巴瘤139例(44.0%)。發(fā)病率位于前兩位的是NK/T細胞淋巴瘤鼻型160例(50.6%)、彌漫大B細胞淋巴瘤64例(20.3%)。NK/T細胞淋巴瘤發(fā)病率高于彌漫大B細胞淋巴瘤。≥60歲的患者106例(33.2%)診斷均為非霍奇金淋巴瘤,其中彌漫大B細胞淋巴瘤40例(37.7%)、NK/T細胞淋巴瘤36例(34.0%),彌漫大B細胞淋巴瘤與NK/T細胞淋巴瘤發(fā)病率無統(tǒng)計學意義。60歲的非霍奇金淋巴瘤患者210例(66.5%),其中NK/T細胞淋巴瘤124例(59.0%)、彌漫大B細胞淋巴瘤24例(11.4%),發(fā)病率差異有顯著性。結(jié)論 316例鼻腔鼻咽部非霍奇金淋巴瘤中以NK/T細胞淋巴瘤最為多見,第二位是彌漫大B細胞淋巴瘤;≥60歲老人鼻腔鼻咽部淋巴瘤患者彌漫大B細胞淋巴瘤、NK/T細胞淋巴瘤最為多見,兩者之間發(fā)病率差異無顯著性。
[Abstract]:Objective to investigate the clinical features, pathological types and types of nasopharyngeal lymphoma.Methods the clinical data, histopathological features and immunophenotype of 319 cases of nasopharyngeal lymphoma in the Department of Pathology of Beijing Friendship Hospital affiliated to Capital Medical University were retrospectively analyzed.The statistical results are compared and analyzed.Results among 319 consultation cases, 3 cases were diagnosed as Hodgkin's lymphoma mixed cell type, and the tumor was involved in nasopharynx.It was suggested that all 316 cases of non-Hodgkin 's lymphoma should be diagnosed as non-Hodgkin 's lymphoma and 99.1%, which included 177 cases of non-Hodgkin 's T and NK cell derived lymphoma (n = 177). 139 cases of B cell derived lymphoma (n = 139) were from B cell origin lymphoma.The incidence of NK-T cell lymphoma was higher in 64 cases of diffuse large B-cell lymphoma than that of diffuse large B-cell lymphoma. The incidence of NK-P T cell lymphoma was higher than that of diffuse large B-cell lymphoma. 106 patients over 60 years of age were diagnosed as non-Hodgkin 's lymphoma.124 cases (59.0%) and 24 cases (11.4%) diffuse large B-cell lymphoma (DSLL) had significant difference in incidence.Conclusion among 316 cases of nasopharyngeal non-Hodgkin 's lymphoma, NK/T cell lymphoma is the most common, the second is diffuse large B cell lymphoma.NK / T cell lymphoma was the most common in patients with nasal and nasopharyngeal lymphoma (鈮，
