【摘要】：目的： 評(píng)估激光治療對(duì)糖尿病黃斑水腫患者視功能的影響。 方法： 對(duì)符合入選標(biāo)準(zhǔn)的23例40只眼糖尿病性視網(wǎng)膜病變患者行專科檢查,眼底熒光血管造影了解水腫情況,在光凝前、光凝后3個(gè)月分別對(duì)患者進(jìn)行光學(xué)相干斷層掃描黃斑中心凹視網(wǎng)膜厚度,并采用MP-1微視野儀對(duì)相同患者行中心10。平均視網(wǎng)膜光敏度、固視穩(wěn)定性(固視穩(wěn)定或固視不穩(wěn)定)、固視位置(中心固視或偏中心固視)檢查。填寫DME癥狀統(tǒng)計(jì)表及觀察表,包括一般資料、全身癥狀、舌脈、國際標(biāo)準(zhǔn)視力表最佳矯正視力、激光治療前、治療后1個(gè)月及3個(gè)月各個(gè)階段MP-1微視野情況等；最后將所有資料錄入數(shù)據(jù)庫,進(jìn)行配對(duì)t檢驗(yàn)、秩和檢驗(yàn)及雙變量相關(guān)進(jìn)行統(tǒng)計(jì)與分析。 結(jié)果： 23個(gè)患者40只眼,激光治療后3個(gè)月視力、平均視網(wǎng)膜光敏度、固視穩(wěn)定性、固視位置及黃斑厚度與激光治療前差異有顯著統(tǒng)計(jì)學(xué)意義；激光治療前后視力、固視穩(wěn)定性、固視位置及平均視網(wǎng)膜光敏度與黃斑厚度呈負(fù)相關(guān),相關(guān)性有統(tǒng)計(jì)學(xué)意義；激光治療前后視力與平均光敏感度呈正相關(guān),相關(guān)性有統(tǒng)計(jì)學(xué)意義；激光治療前后固視位置及平均光敏度與固視穩(wěn)定性呈正相關(guān),相關(guān)性有統(tǒng)計(jì)學(xué)意義；中醫(yī)證型與激光前后視力無相關(guān)性。 結(jié)論： 資料顯示,激光對(duì)糖尿病性黃斑水腫患者視力、黃斑厚度、視網(wǎng)膜光敏感度、固視穩(wěn)定性、固視位置在治療后3個(gè)月有明顯改善作用。
[Abstract]:Objective:To evaluate the effect of laser therapy on visual function in diabetic macular edema patients.Methods:Twenty three patients (40 eyes) with diabetic retinopathy who met the inclusion criteria were examined by special examination, fundus fluorescein angiography (FFA) was performed to find out the edema, and before photocoagulation,The retinal thickness of macular central fovea was scanned by optical coherence tomography 3 months after photocoagulation, and the center of the same patient was measured by MP-1 microfield.The mean retinal Guang Min degree, fixation stability (fixed vision stability or fixed vision instability), fixed vision position (central fixation or partial central fixation) were examined.Filling out DME symptom statistics and observation forms, including general information, systemic symptoms, tongue vein, international standard visual acuity, MP-1 microvisual field of each stage before, 1 month and 3 months after laser treatment, etc.Finally, all the data are entered into the database and analyzed by paired t test, rank sum test and bivariate correlation.Results:The visual acuity, mean retinal Guang Min degree, fixation stability, fixation position and macular thickness of 23 eyes were significantly different from those before and after laser treatment.The position of fixed vision and the mean retinal Guang Min degree were negatively correlated with macular thickness, the visual acuity was positively correlated with mean light sensitivity before and after laser treatment, and the correlation was statistically significant.There was a positive correlation between the fixed vision position and the average Guang Min degree before and after laser treatment, and the correlation was statistically significant, while there was no correlation between the type of TCM syndrome and the visual acuity before and after laser treatment.Conclusion:The results showed that laser could significantly improve the visual acuity, macular thickness, retinal light sensitivity, fixation stability and fixation position of diabetic macular edema patients 3 months after treatment.
