新生兒聽力篩查中聯(lián)合應(yīng)用DPOAE和AABR的回顧性分析

【摘要】:目的 :初步分析畸變產(chǎn)物耳聲發(fā)射(distortion product otoacoustic emission,DPOAE)聯(lián)合自動聽性腦干反應(yīng)(automated auditory brainstem response,AABR)在新生兒聽力篩查中的應(yīng)用。方法:回顧性分析2012年5月—2013年5月新生兒在本院聽力篩查結(jié)果,新生兒出生后3~4 d進(jìn)行DPOAE初篩,出生后42 d進(jìn)行復(fù)篩,進(jìn)行復(fù)篩時,初篩通過者應(yīng)用AABR檢查,未通過者復(fù)查DPOAE的同時進(jìn)行AARB檢查。復(fù)篩中有任一項(xiàng)檢查未通過者接受聽力學(xué)診斷評估,包括聽性腦干反應(yīng)測試技術(shù)(auditory brainstem response,ABR)和聲導(dǎo)抗檢查。結(jié)果:共7 569例進(jìn)行了DPOAE和AABR的聯(lián)合篩查,其中有191例進(jìn)行了ABR檢查,AABR與ABR的一致率為91.62%,DPOAE與ABR的一致率為57.07%,而DPOAE聯(lián)合AABR篩查與ABR的一致率為66.67%。在191例ABR檢查中127例(213耳)存在聽力異常,其中輕度聽力損失72耳(33.80%),中度聽力損失54耳(25.35%),重度聽力損失25耳(11.74%),極重度聽力損失62耳(29.11%)。結(jié)論:聯(lián)合應(yīng)用DPOAE和AABR可以降低新生兒聽力異常的漏診、誤診率,對于聽力障礙患兒的早期發(fā)現(xiàn)、早期診斷、早期治療具有重要的意義。
[Abstract]:Objective: to analyze the application of distortion product otoacoustic emission (distortion product otoacoustic emission) combined with automatic auditory brainstem response (AABR) in neonatal hearing screening.Methods: the results of neonatal hearing screening in our hospital from May 2012 to May 2013 were retrospectively analyzed. DPOAE screening was performed 3 days after birth and 42 days after birth. When screening was performed, AABR examination was applied to those who passed the screening.AARB was performed at the same time as DPOAE was rechecked by those who failed.Any of the patients who failed the screening were assessed for audiological diagnosis, including auditory brainstem response (ABR) and acoustic impedance (ABR).緇撴灉:鍏,
