阻塞性睡眠呼吸暫停低通氣綜合征患者血紅蛋白、EPO及EPOR水平變化研究

【摘要】:目的:測定并比較阻塞性呼吸暫停低通氣綜合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者和健康對照組的紅細(xì)胞計(jì)數(shù)、血紅蛋白、紅細(xì)胞壓積、促紅細(xì)胞生成素及促紅細(xì)胞生成素受體水平,進(jìn)一步探討OSAHS與紅細(xì)胞增生關(guān)系及發(fā)生機(jī)制。方法:收集2015年12月~2016年12月到青海大學(xué)附屬醫(yī)院行多導(dǎo)睡眠監(jiān)測并確診為OSAHS的41例漢族男性患者為病例組,平均年齡為(49.7±9.5)歲,并選取同時期到青海大學(xué)附屬醫(yī)院體檢者32例漢族男性為對照組,平均年齡為(45.4±11.1)歲,經(jīng)PSG檢查后AHI5次/h,抽取清晨靜脈血,測定并比較兩組紅細(xì)胞計(jì)數(shù)、血紅蛋白、紅細(xì)胞壓積和睡眠監(jiān)測指標(biāo),采用雙抗體一步夾心法酶聯(lián)免疫吸附實(shí)驗(yàn)法(ELISA)測定血清EPO、EPOR水平并比較。結(jié)果:1.病例組紅細(xì)胞計(jì)數(shù)、血紅蛋白、紅細(xì)胞壓積顯著高于對照組[分別為5.50(5.18,5.96)vs.5.12(5.01,5.23),P=0.000;(175.5±25.0)vs.(156.9±3.8),P=0.000;50.10(47.00,54.90)vs.48.00(47.23,49.08),P=0.003],病例組和對照組EPO、EPOR無顯著性差異[3.0(1.8,3.4)vs.2.6(2.0,3.9),P=0.523;6.9(5.2,8.1)vs.5.3(3.7,7.6),P=0.102],血紅蛋白與氧減指數(shù)(r=0.321,P=0.041)、Sp O290%時間占總睡眠時間百分比(r=0.539,P=0.000)正相關(guān),與最低血氧飽和度(r=-0.387,P=0.012)、平均血氧(r=-0.526,P=0.000)負(fù)相關(guān),但與EPO、EPOR無顯著線性相關(guān)。2.兩組EPO與睡眠指標(biāo)均無相關(guān)關(guān)系。結(jié)論:1.OSAHS患者紅細(xì)胞計(jì)數(shù)、血紅蛋白、紅細(xì)胞壓積水平明顯高于健康對照組,血紅蛋白與低氧密切相關(guān),但與清晨促紅細(xì)胞生成素、促紅細(xì)胞生成素受體無顯著線性相關(guān)。2.OSAHS患者清晨血清樣本不能充分反映夜間的病理生理變化。
[Abstract]:Objective: to determine and compare the erythrocyte count, hemoglobin, hematocrit, erythropoietin and erythropoietin receptor levels in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS) and healthy controls.To further explore the relationship between OSAHS and erythrocyte proliferation and its mechanism.Methods: from December 2015 to December 2016, 41 male Han Chinese patients with OSAHS who were admitted to the affiliated Hospital of Qinghai University for polysomnography were collected as the case group, with an average age of 49.7 鹵9.5 years.At the same time, 32 Han nationality males from the affiliated Hospital of Qinghai University were selected as the control group. The average age was 45.4 鹵11.1 years old. After PSG examination, AHI5 times / h was taken out, and the red blood cell count and hemoglobin were measured and compared between the two groups.The levels of EPO-EPOR in serum were measured by double antibody one step sandwich enzyme-linked immunosorbent assay (Elisa) with hematocrit and sleep monitoring index.The result is 1: 1.There was a negative correlation with the lowest oxygen saturation (r = 0.387) and an average oxygen oxygen (n = 0. 526), but no significant linear correlation with EPOR.There was no correlation between EPO and sleep index in both groups.Conclusions 1. The erythrocyte count, hemoglobin and hematocrit in OSAHS patients were significantly higher than those in healthy controls. Hemoglobin was closely related to hypoxia, but it was associated with erythropoietin in the early morning.There was no significant linear correlation between erythropoietin receptor and erythropoietin receptor. 2. The early morning serum samples of OSAHS patients could not fully reflect the nocturnal pathophysiological changes.
