【摘要】：目的：青光眼是全世界范圍內的主要致盲性眼病之一。雖然其病因至今尚未完全清楚，但社會心理因素在青光眼的臨床診療過程中的作用逐漸引起了人們的關注�；颊甙l(fā)病前經歷的負性生活事件產生的情緒應激是導致青光眼患者發(fā)病的重要原因之一，青光眼的發(fā)病及復發(fā)與精神、情緒等有關，反過來心理情緒的變化又影響了青光眼的治療。本文通過調查大連市急性閉角型青光眼患者的心理、個性和行為特征，以達到對大連市目前青光眼患者心理狀態(tài)的一個基本認識，并通過與正常人比較，分析原發(fā)性急性閉角型青光眼(PACG)患者的心理、個性和行為特征及其與疾病的關系。 方法：選取2011年12月-2012年3月在大連醫(yī)科大學附屬第一醫(yī)院眼科門診及病房確診的PACG患者53例作為病例組，健康體檢者55例作為對照組。對53例PACG患者和55例健康體檢者分別運用癥狀自評量表(SCL-90)，生活事件量表（LES），艾森克個性間卷(EPQ)和A型行為問卷(TABQ)進行調查評估。 結果：(1)SCL-90評分比較，，青光眼組的軀體化、強迫、人際關系、抑郁、焦慮、恐怖、偏執(zhí)、精神病性等癥狀因子評分和SCL-90總分均明顯高于對照組(P0.01)。(2) A型行為問卷測評結果，青光眼組TH量表得分高于正常組，有明顯差異，TH+CH和CH得分兩組間無明顯差異。(3)艾森克個性問卷中青光眼組的精神質(P)、神經質或情緒(N)得分明顯高于對照組(P 0.05)，而內外向、掩飾兩項得分青光眼與對照組無顯著性差異。(4) LES量表結果分析發(fā)現，青光眼組在家庭事件、社會及其他事件、生活事件總分以及壓力反應總分方面得分均高于對照組，差異具有統(tǒng)計學意義（P＜0.05），而在工作學習事件和經濟事件方面得分差異不顯著。(5)社會因素及個性特征與心理健康水平的相關性分析:年齡和文化與軀體化癥狀(fl)成正相關,家庭滿意度與與軀體化癥狀(fl)成負相關,職業(yè)和家庭滿意度與人際關系敏感(f3)成負相關,神經質或情緒(N)與抑郁(f4)成正相關，家庭滿意度與焦慮(f5)成負相關。 結論：社會心理因素、個性與原發(fā)性急性閉角型青光眼有明確的聯(lián)系，青光眼患者主要表現為A型性格，患者的精神性和神經質傾向人格特征與閉角型青光眼發(fā)病密切相關。
[Abstract]:Objective: glaucoma is one of the major blinding eye diseases worldwide. However, the role of psychosocial factors in the clinical diagnosis and treatment of glaucoma has gradually attracted people's attention. Emotional stress caused by negative life events before onset of glaucoma is one of the important reasons leading to the onset of glaucoma. The onset and recurrence of glaucoma are related to mental and emotional factors. In turn, the changes of psychological emotions affect the treatment of glaucoma. This paper investigates the psychological, personality and behavioral characteristics of patients with acute angle-closure glaucoma in Dalian. In order to reach a basic understanding of the psychological state of current glaucoma patients in Dalian, and to analyze the psychological, personality and behavioral characteristics of patients with primary acute angle-closure glaucoma (PACGG) and their relationship with diseases through comparison with normal persons. Methods: from December 2011 to March 2012, 53 PACG patients were selected from the first affiliated Hospital of Dalian Medical University as the case group. A total of 53 PACG patients and 55 healthy controls were investigated with SCL-90, life events scale, Eysenck personality test (EPQ) and type A behavior questionnaire (TABQ). Results the scores of somatization, obsessive-compulsion, interpersonal relationship, depression, anxiety, phobia, paranoid ideation, psychosis and total score of SCL-90 in the glaucoma group were significantly higher than those in the control group (P 0.01). The score of th scale in glaucoma group was higher than that in normal group, and there was no significant difference in th Ch and Ch scores between the two groups. There was no significant difference between the two groups. The scores of psychosis, neuroticism or mood in Eysenck personality questionnaire were significantly higher than those in the control group (P 0.05, P 0.05), while the scores in and out of the glaucoma group were higher than those in the control group (P 0.05). The results of LES scale showed that the scores of family events, social and other events, total scores of life events and total scores of stress response in glaucoma group were higher than those in control group. The difference was statistically significant (P < 0.05), but there was no significant difference in scores of work and study events and economic events (P < 0.05). Analysis of the correlation between social factors and personality characteristics and mental health level: age and culture were positively correlated with somatization symptoms. Family satisfaction was negatively correlated with somatization, career and family satisfaction and interpersonal sensitivity f3), neuroticism or emotion NN) was positively correlated with depression f4), family satisfaction was negatively correlated with anxiety f5). Conclusion: psycho-social factors, personality and primary acute angle closure glaucoma are closely related to the onset of angle closure glaucoma. The main manifestations of glaucoma patients are type A personality. The mental and neurotic personality characteristics of the patients are closely related to the onset of angle closure glaucoma.
