【摘要】：目的探討早期吞咽功能訓(xùn)練對鼻咽癌放療患者功能恢復(fù)及生活質(zhì)量的影響。方法選取2010年1月至2015年7月間接受治療的52例鼻咽癌放療患者,根據(jù)隨機(jī)數(shù)字法將其分為觀察組和對照組,每組26例。觀察組患者在常規(guī)治療基礎(chǔ)上,早期加用吞咽功能訓(xùn)練,對照組患者僅采用常規(guī)治療。比較兩組患者治療前后吞咽功能的變化情況,以及放療的臨床療效、吸入性肺炎發(fā)生率。根據(jù)生活質(zhì)量測定量表(EORTC-QLQ-C30),評估和比較治療前后患者生活質(zhì)量各維度得分及總分變化情況。結(jié)果與對照組比較,觀察組患者治療后吞咽功能得到了明顯改善,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。與對照組比較,觀察組患者總有效率明顯提高,吸入性肺炎發(fā)生率顯著降低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。與對照組比較,觀察組患者治療后生活質(zhì)量各維度得分及總分均明顯增高,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論早期吞咽功能訓(xùn)練有助于鼻咽癌放療患者吞咽功能的恢復(fù),明顯提高放療的臨床療效及生活質(zhì)量。
[Abstract]:Objective to investigate the effect of early swallowing function training on functional recovery and quality of life (QOL) of patients with nasopharyngeal carcinoma (NPC) after radiotherapy. Methods 52 patients with nasopharyngeal carcinoma were treated indirectly from January 2010 to July 2015. According to the random number method, the patients in the observation group were divided into two groups, the observation group and the control group, with 26 cases in each group. The patients in the observation group were treated with swallowing function training at the early stage on the basis of routine treatment. The patients in the control group were treated with routine therapy only. The changes of swallowing function before and after treatment and the clinical efficacy of radiotherapy were compared between the two groups. Incidence of aspiration pneumonia. According to EORTC-QLQ-C30, the patients' quality of life scores and total scores before and after treatment were evaluated and compared. Results compared with the control group, the swallowing function of the patients in the observation group was significantly improved after treatment. Compared with the control group, the total effective rate of the patients in the observation group was significantly increased, the incidence of aspiration pneumonia was significantly decreased, and the difference was statistically significant compared with the control group. The scores and total scores of quality of life in the observation group were significantly higher than those in the control group (P 0.05). Conclusion the early swallowing function training is helpful to the recovery of swallowing function in patients with nasopharyngeal carcinoma after radiotherapy. The clinical curative effect and quality of life of radiotherapy were improved obviously.
