【摘要】：目的探討治療前血清球蛋白水平對(duì)評(píng)估鼻咽癌預(yù)后的價(jià)值。方法回顧性收集2009年1月至2013年12月在南方醫(yī)院初治的127例無(wú)遠(yuǎn)處轉(zhuǎn)移鼻咽癌患者資料,包括治療前血清球蛋白水平。采用ROC曲線選取血清球蛋白分界值從而將病例分成低球蛋白組和高球蛋白組,采用Kaplan-Meier和COX多因素分析的方法分析治療前血清球蛋白水平對(duì)判斷鼻咽癌預(yù)后的價(jià)值。結(jié)果 ROC曲線得出治療前血清球蛋白的分界值為30.05 g/L。球蛋白水平與性別及N分期顯著相關(guān)。Kaplan-Meier分析顯示治療前高球蛋白水平(30.05 g/L)預(yù)示不良的無(wú)進(jìn)展生存率(P=0.019),總生存率(P=0.034),及無(wú)遠(yuǎn)處轉(zhuǎn)移生存率(P=0.049)。多因素分析顯示治療前血清球蛋白水平是鼻咽癌進(jìn)展的獨(dú)立預(yù)后因素(HR=2.344,P=0.031)。結(jié)論治療前血清球蛋白水平對(duì)鼻咽癌的預(yù)后評(píng)估有臨床價(jià)值。
[Abstract]:Objective to evaluate the value of serum globulin level in evaluating the prognosis of nasopharyngeal carcinoma (NPC) before treatment. Methods the data of 127 patients with nasopharyngeal carcinoma without distant metastasis were collected retrospectively from January 2009 to December 2013. The ROC curve was used to select the serum globulin boundary value to divide the patients into low globulin group and high globulin group. Kaplan-Meier and COX multivariate analysis were used to analyze the value of serum globulin level in predicting prognosis of nasopharyngeal carcinoma before treatment. Results ROC curve showed that the boundary value of serum globulin before treatment was 30.05g / L 路globulin level, sex and N. Kaplan-Meier analysis showed that high globulin level (30.05 g / L) predicted poor progressive survival rate (P < 0.019), overall survival rate (P < 0.034), and metastasis free survival rate (P < 0.049). Multivariate analysis showed that the serum globulin level before treatment was nasopharyngeal carcinoma (NPC). Conclusion the serum globulin level before treatment is of clinical value in evaluating the prognosis of nasopharyngeal carcinoma.
