【摘要】：目的通過探討穿透性角膜移植術后外傷性傷口裂開的患者特征、危險因素、并發(fā)癥和視覺結(jié)果,提高穿透性角膜移植術后對外傷的重視及防護。方法回顧性分析我院2014-2015年8例8眼外傷致穿透性角膜移植傷口裂開患者并同期所有英文文獻類似報道病歷資料49例49眼,共57例57眼。結(jié)果57例57眼外傷時年齡4-72歲,平均年齡39.86±17.63歲。穿透性角膜移植術和外傷之間的平均時間23.05±15.99月,范圍0.6月-34年。在眼前段損傷中,晶狀體/人工晶體脫出26眼、虹膜脫垂22眼、前房積血19眼。在眼后段損傷中,玻璃體脫出18眼、玻璃體積血17眼、視網(wǎng)膜脫離8眼、脈絡膜上腔出血3眼。所有患者中外傷后繼發(fā)眼內(nèi)炎、增殖性玻璃體視網(wǎng)膜病變各1眼。再次治療后最佳矯正視力21眼≥0.1,13眼手動至0.1,15眼手動或者光感,8眼無光感。四肢及不同物體擊傷眼部是本研究中傷口裂開的最常見原因。結(jié)論穿透性角膜移植術后傷口裂開的風險終身存在,無論患者年齡、手術適應癥、穿透性角膜移植術后時間,尤其是外傷后更容易發(fā)生且傷口裂開時間最可能發(fā)生在穿透性角膜移植術后第一年。多數(shù)患者有較差的視覺結(jié)果,少數(shù)無嚴重眼部并發(fā)癥且無眼后段損傷者可有良好視覺結(jié)果。
[Abstract]:Objective to investigate the characteristics, risk factors, complications and visual results of traumatic wound rupture after penetrating keratoplasty. Methods 8 cases (8 eyes) with penetrating keratoplasty wound rupture caused by penetrating keratoplasty in our hospital from 2014 to 2015 and 49 cases 49 eyes (49 eyes) with similar medical records reported in the same period were analyzed retrospectively. Results 57 cases (57 eyes) were aged 4-72 years (mean age 39.86 鹵17.63). The mean time between penetrating keratoplasty and trauma was 23.05 鹵15.99 months, ranging from 0.6 months to 34 years. In the anterior segment injury, 26 eyes were out of lens / intraocular lens. Iris prolapse in 22 eyes, hyphema in 19 eyes, vitreous prolapse in 18 eyes, vitreous hemorrhage in 17 eyes, retinal detachment in 8 eyes, suprachoroidal hemorrhage in 3 eyes. 1 eye each with proliferative vitreoretinopathy. The best corrected visual acuity after retreatment was 21 eyes 鈮，
