【摘要】：目的研究膽脂瘤患者顳骨CT解剖參數(shù)與膽脂瘤的關(guān)系及意義。方法回顧性分析經(jīng)手術(shù)病理證實(shí)咽鼓管上隱窩(STR)骨性結(jié)構(gòu)完整的膽脂瘤患者87例(一側(cè)患病對(duì)側(cè)正常,設(shè)為病例組)的MSCT資料,將其分成病例組患側(cè)組、病例組健側(cè)組;40名正常人設(shè)為對(duì)照組。測(cè)量咽鼓管外耳道角(ATA)、STR的橫徑(T)、外耳道上棘乙狀竇間距離(H-SS)、齒突長(zhǎng)度(Cog)。結(jié)果對(duì)照組、病例組健側(cè)組、患側(cè)組ATA分別為(149.63±9.39)°、(147.13±4.90)°、(145.86±5.80)°,T分別為(3.56±0.68)mm、(3.43±0.55)mm、(3.01±0.72)mm,Cog分別為(1.71±0.34)mm、(1.68±0.44)mm、(1.31±0.36)mm,H-SS分別為(13.9±2.24)mm、(12.69±2.96)mm、(11.82±3.53)mm。對(duì)照組左右側(cè)比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05);對(duì)照組與病例組患側(cè)組ATA、T、Cog、H-SS差異均有統(tǒng)計(jì)學(xué)意義(P0.05);病例組患側(cè)組與健側(cè)組僅Cog、T差異有統(tǒng)計(jì)學(xué)意義(P0.05),對(duì)照組與病例組健側(cè)比較僅H-SS差異有統(tǒng)計(jì)學(xué)意義(P0.05)。對(duì)照組的ATA/H-SS、Cog/T、H-SS/T有低度相關(guān)性(r=0.39,-0.38,0.36;P0.05);病例組ATA/H-SS有低度相關(guān)性(r=-0.307,P0.05),其余各組數(shù)據(jù)均無相關(guān)性。結(jié)論顳骨各解剖參數(shù)構(gòu)成一個(gè)整體,具有一定相關(guān)性且相互影響,部分解剖參數(shù)異常及各參數(shù)間關(guān)系失衡或使一些患者易于罹患膽脂瘤。
[Abstract]:Objective to study the relationship between CT anatomical parameters of temporal bone and cholesteatoma in patients with cholesteatoma. The MSCT data were divided into two groups: the diseased side of the case group and the healthy side of the case group. 40 normal persons in the healthy side group were set as the control group. The transverse diameter of the external auditory canal angle of the eustachian tube, the distance between the superior sigmoid sinus of the external auditory canal and the length of the odontoid process were measured. Results in the control group, the length of the odontoid process was Cog.Results in the control group, Case group, healthy side group, The ATA of the affected side was 149.63 鹵9.39 擄and 147.13 鹵4.90 擄respectively 145.86 鹵5.80 擄T was 3.56 鹵0.68 鹵0.55 鹵3.43 鹵0.55 鹵0.52 鹵3.01 鹵0.72 鹵0.32 鹵1.71 鹵0.34 mm respectively. The difference of ATA between the two groups was 13.9 鹵2.24 鹵12.69 鹵2.96 鹵11.82 鹵3.53mm / mm. there was no significant difference between the control group and the control group, and there was significant difference between the control group and the control group in ATAT and CogH-SS, and there was no significant difference in the left and right sides between the two groups (P < 0.05). The difference of Cogus T was statistically significant (P 0.05). The difference of H-SS between the control group and the case group was only significant (P 0.05). There was a low correlation between the control group and the control group. There was a low correlation between the ATA/H-SS of the control group and that of the control group. Conclusion there was no correlation between the data of the other groups and the ATA/H-SS of the case group. The anatomical parameters of the bone form a whole. Some of the anatomical parameters are abnormal and the relationship between the parameters is out of balance or some patients are prone to cholesteatoma.
