【摘要】：目的評(píng)估改良咽鼓管吹張治療兒童分泌性中耳炎的可行性、安全性、有效性。方法將我科門診確診為分泌性中耳炎的患兒隨機(jī)分為四組,對(duì)照組不接受任何處理,觀察等待3個(gè)月,藥物治療組使用鼻噴劑+口服抗組胺藥+口服粘液促排劑治療1個(gè)月后繼續(xù)觀察2個(gè)月,咽鼓管吹張組使用改良咽鼓管吹張器治療1個(gè)月后繼續(xù)觀察2個(gè)月,藥物+咽鼓管吹張組同時(shí)使用藥物+咽鼓管吹張治療1個(gè)月后繼續(xù)觀察2個(gè)月。結(jié)果治療過(guò)程中接受咽鼓管吹張的患兒均能配合完成治療,無(wú)嚴(yán)重的不良反應(yīng)和不適。治療1個(gè)月后四組的療效差異有統(tǒng)計(jì)學(xué)意義,吹張組和藥物+吹張組有效率高于對(duì)照組和藥物治療組,繼續(xù)觀察2個(gè)月后這種差異仍然存在。吹張組和藥物+吹張組之間療效無(wú)統(tǒng)計(jì)學(xué)差異。治療后吹張組和藥物+吹張組氣骨導(dǎo)差值明顯下降,觀察組和藥物組治療前后氣骨導(dǎo)差值無(wú)變化。結(jié)論改良咽鼓管吹張治療兒童分泌性中耳炎是可行的安全的有效的,在觀察等待的3個(gè)月中可有效清除中耳積液,降低需要手術(shù)的機(jī)率。
[Abstract]:Objective to evaluate the feasibility of modified eustachian tube in treatment of secretory otitis media in children's safety, effectiveness. Methods in our department diagnosed patients were randomly divided into secretory otitis media in four groups, the control group did not receive any treatment, observation wait 3 months, drug treatment group using nasal spray + oral antihistamines oral mucous + eccritic after treatment for 1 months to 2 months of observation, the eustachian tube group using modified eustachian tube for 1 months after treatment to observe for 2 months, the drug + eustachian tube group and drug + eustachian tube after treatment for 1 months to 2 months of observation. The treatment results in the process of acceptance of the eustachian tube in children are in line with the completion of treatment, no serious adverse reactions and discomfort. There was statistical significance after 1 months of treatment effect differences between the four groups, blowing group and drug + blowing group effective rate higher than that of control group and treatment group, following the Continue to observe this difference after 2 months still exist. The curative effect between the group and a blowing drug + blowing group no significant difference. After treatment, blowing group and drug group + blowing air bone difference decreased, observed before and after treatment group and drug group air bone difference change. Conclusion eustachian tube treatment secretory otitis media in children is feasible and safe and effective, can effectively remove the middle ear effusion in 3 months were waiting, reducing the probability of surgery.
