【摘要】：目的觀察視網(wǎng)膜母細(xì)胞瘤(retinoblastoma,RB)患者全身化學(xué)藥物治療聯(lián)合局部治療的臨床效果。方法臨床納入RB患者19例,共有28只眼接受了全身化學(xué)藥物治療聯(lián)合局部治療。全身化療一般進(jìn)行6個療程,每個療程3~4周,主要化療藥物包括長春新堿、環(huán)磷酰胺、依托泊甙以及卡鉑等。此外,根據(jù)患者的情況給予局部治療。局部治療方法包括激光光凝、冷凍、經(jīng)瞳孔溫?zé)岑煼�、�?06放射敖貼器局部放療以及眼球摘除等。對患者進(jìn)行3~61個月的隨訪,平均隨訪時間為31個月。觀察患者的臨床生存率、眼球保留率以及臨床療效。結(jié)果隨訪1年內(nèi),有16例患者存活,存活率為84.21%,轉(zhuǎn)移率為5.26%;2年內(nèi)生存率為78.95%,轉(zhuǎn)移率為10.53%。對16例存活患者23只患眼進(jìn)行分析,發(fā)現(xiàn)腫瘤分期為Ⅰ~Ⅱ期、Ⅲ~Ⅳ、Ⅴ期者眼球保留率分別為100.00%、40.00%、16.67%。3例死亡病例均為腫瘤分期Ⅳ期及以上的患者。結(jié)論 RB患者進(jìn)行全身化學(xué)藥物聯(lián)合局部治療,具有較好的臨床療效,早期治療患者患眼的保留率較高。
[Abstract]:Objective to observe the clinical effect of systemic chemotherapeutic therapy combined with local therapy in patients with retinoblastoma (RBB). Methods 19 patients with RB were included in this study. A total of 28 eyes received systemic chemotherapeutic therapy combined with local treatment. General systemic chemotherapy was performed for 6 courses of treatment for 3 to 4 weeks each. The main chemotherapeutic drugs included vincristine, cyclophosphamide, etoposide, carboplatin, and so on. Local treatment was given according to the patient's condition. The local treatment methods included laser photocoagulation, freezing, transpupillary thermotherapy, local radiotherapy with ruthenium 106 radiator and enucleation, etc. The patients were followed up for 3 to 61 months. The average follow-up time was 31 months. The clinical survival rate, eyeball retention rate and clinical efficacy were observed. The survival rate was 84.21 and the metastasis rate was 5.26.
