【摘要】：目的：建立1~5歲正常兒童內(nèi)耳系統(tǒng)各主要結(jié)構(gòu)的MRI測量正常值范圍。 材料與方法：本論文選取了26例1~5歲健康兒童志愿者，共52耳行MRI水成像檢查，將所有原始圖像傳送至工作站后進(jìn)行三維重建，，多角度旋轉(zhuǎn)并測量內(nèi)耳各主要結(jié)構(gòu)的徑線值，并將各測量指標(biāo)就性別、側(cè)別間進(jìn)行比較。 結(jié)果：(1)MRI內(nèi)耳水成像技術(shù)可清晰顯示正常內(nèi)耳膜迷路各主要結(jié)構(gòu)；(2)三條半規(guī)管高度性別間差異均有顯著性意義(P0.05)，分別為上半規(guī)管高度男性(5.86±0.33)mm與女性(5.65±0.11)mm；后半規(guī)管高度男性(5.68±0.33)mm與女性(5.38±0.40)mm；水平半規(guī)管高度男性(4.04±0.34)mm與女性(3.77±0.30)mm。其余測量指標(biāo)性別間差異均無顯著性意義(P0.05)。(3)所有測量指標(biāo)側(cè)別間差異均無顯著性意義。(4)95%雙側(cè)范圍分別為耳蝸高度(3.10~3.92)mm、底徑(3.85~5.37)mm；前庭長徑(4.94~5.96)mm、短徑(3.02~4.24)mm；上半規(guī)管高度男性(5.02~6.70)mm與女性(4.87~6.43)mm、跨度(3.81~5.33)mm、管徑(0.24~0.86)mm；后半規(guī)管高度男性(4.37~6.99)mm與女性(4.36~6.40)mm、跨度(1.83~3.05)mm、管徑(0.26~1.12)mm；水平半規(guī)管高度男性(3.28~4.80)mm與女性(2.97~4.57)mm、跨度(3.26~4.60)mm、管徑(0.26~0.92)mm。 結(jié)論：以幼兒內(nèi)耳MR測量正常值范圍的建立為基礎(chǔ)，可以進(jìn)一步完善先天性內(nèi)耳畸形的診斷和分類方法，這對于幼兒耳聾的早期診斷和人工耳蝸植入術(shù)的成功實(shí)施都具有重要意義。
[Abstract]:Objective: to establish a range of MRI measurements of the main structures of the inner ear system in 5 years old children. Materials and methods: in this study, 52 ears of 26 healthy children aged 1 to 5 years were examined by MRI hydrography. All the original images were transferred to the workstation for 3D reconstruction, and the diameter of the main structures of the inner ear was measured and rotated at multiple angles. The measurement indexes were compared with each other in terms of sex and side. Results there were significant differences in the height of the three semicircular canals between men and women (5.86 鹵0.33mm in upper semicircular canal and 5.65 鹵0.11mm in female). The horizontal semicircular canal height was 4.04 鹵0.34mm in males and 3.77 鹵0.30mmmmmmmm. there was no significant difference in other measurements between males and females. The length of vestibular vestibular vestibule is 4.94 / 5.96mm, and the short diameter is 3.024.24mm; the upper semicircular canal height is 5.026.70mm for men and 4.876.43mm for women, with a span of 3.815.33mm and 0.240.86mm; the posterior semicircular canal height is 4.376.99mm for males and 4.366.40mm for females, and 1.833.05mm for diameter 0.261.12mm; the horizontal semicircular canal height is 3.284.80mm for males and 2.974.57mm for females, with a span of 0.2620.9mm. Conclusion: the diagnosis and classification of congenital inner ear malformation can be further improved on the basis of the establishment of normal range of Mr measurement in infants' inner ear. This is of great significance for the early diagnosis of deafness and the successful implementation of cochlear implantation.
