【摘要】：目的探討視網(wǎng)膜靜脈阻塞致病因素及影響預后視力的相關因素,為視網(wǎng)膜靜脈阻塞患者的臨床治療提供參考。方法收集2014年5月至2015年5月鎮(zhèn)安縣醫(yī)院治療并確診為視網(wǎng)膜靜脈阻塞患者170例,依據(jù)患者病灶部位,將其分為總干阻塞(n=47)、半側阻塞(n=68)及分支阻塞(n=55)。對患者的臨床資料進行回顧性分析。結果視網(wǎng)膜靜脈阻塞致病原因Logistic回歸分析數(shù)據(jù)顯示,高血壓(OR:4.161;95%CI:2.234~7.749)和高密度脂蛋白(HDL)降低(OR:12.308;95%CI:5.945~25.478)與視網(wǎng)膜靜脈阻塞發(fā)病呈正相關(P=0.000)。而糖尿病、高脂血癥、高TG、高Tc及青光眼與視網(wǎng)膜靜脈阻塞的發(fā)病差異無統(tǒng)計學意義(P0.05)。長期吸煙可能為視網(wǎng)膜靜脈阻塞的保護因素。治療后,與初診視力相比,三種類型視網(wǎng)膜阻塞患者預后視力均有明顯提高,差異有統(tǒng)計學意義(P0.05);視網(wǎng)膜靜脈阻塞患者初診視力對預后視力影響很大,患者初診視力越高,其預后視力效果越好,差異有統(tǒng)計學意義(P0.05)。結論缺血型視網(wǎng)膜靜脈阻塞是影響患者視力預后的主要因素;患者視力預后與其初診水平有很大關系。
[Abstract]:Objective to investigate the pathogenic factors of retinal vein occlusion (RVO) and the related factors affecting the prognosis of visual acuity. Methods from May 2014 to May 2015, 170 patients with retinal vein occlusion were treated in Zhenan County Hospital and diagnosed as retinal vein occlusion. The patients were divided into total trunk occlusion (47), hemiocclusion (68) and branch occlusion (55). The clinical data of the patients were retrospectively analyzed. Results the Logistic regression analysis data of the causes of retinal vein occlusion were analyzed. Hypertension: OR4.161 / 95CI: 2.2347.749) and high density lipoprotein (HDL) lowering OR12.308 and 95 CI: 5.945 ~ 25.478)) there is a positive correlation between the incidence of retinal vein occlusion and diabetes mellitus and hyperlipidemia. There was no significant difference in the incidence of high TG, high Tc and retinal vein occlusion between glaucoma and retinal vein occlusion (P 0.05). Long-term smoking may be the protective factor for retinal vein occlusion. The prognosis visual acuity of the three types of retinal obstruction patients was significantly improved, the difference was statistically significant (P 0.05), the primary visual acuity of the patients with retinal vein occlusion had a great influence on the prognosis visual acuity, the higher the first diagnosis visual acuity of the patients, the better the prognosis visual acuity effect. Conclusion Ischemic retinal vein occlusion is the main factor affecting the prognosis of visual acuity, and the prognosis of visual acuity is closely related to the level of initial diagnosis.
