【摘要】：目的探討纖維母細胞性網(wǎng)狀細胞腫瘤(fibroblastic reticular cell tumor,FRCT)的臨床病理學特點、診斷及鑒別診斷。方法回顧性分析1例扁桃體FRCT的臨床病理學特征并復(fù)習相關(guān)文獻。結(jié)果患者行右側(cè)扁桃體腫物活檢,鏡下見扁桃體正常結(jié)構(gòu)被破壞,腫瘤細胞增生,腫瘤細胞梭形、多邊形、圓形或卵圓形,呈束狀、漩渦狀或席紋狀排列,腫瘤細胞間可見多量散在的嗜酸性粒細胞和中性粒細胞,腫瘤細胞邊界不清,細胞核呈空泡狀,核膜光滑或不規(guī)則,細胞核有輕~中度異型性,可見核仁,核分裂易見(10/50 HPF)。免疫表型:腫瘤細胞Kp-1、vimentin均陽性,CK(AE1/AE3)和SMA均灶狀陽性,S-100蛋白弱陽性,CD20、ALK、CD3、CD23、CD21、CD35、CD34、desmin、p63、CK5/6和CKL均陰性,EBER原位雜交檢測陰性。結(jié)論 FRCT臨床較少見,發(fā)生于扁桃體者更為少見,診斷主要依賴病理學及免疫表型。
[Abstract]:Objective to investigate the clinicopathological features of fibroblastic reticular cell tumors (FRCTs). Methods the clinicopathological features of a patient with tonsil FRCT were retrospectively analyzed and the relevant literatures were reviewed. Results the normal structure of tonsil was destroyed and tumor cells proliferated. Tumor cells are fusiform, polygonal, round or oval, arranged in bundles, whirlpools or sheets. A large number of scattered eosinophils and neutrophils can be seen between tumor cells. The boundaries of tumor cells are unclear, and the nuclei are vacuolated. The nuclear membrane is smooth or irregular, the nucleus has mild to moderate heterogeneity, the nucleolus can be seen. The immunophenotype: Kp-1vimentin was positive for KP-1 / AE3) and SMA was both focal positive and weakly positive for S-100 protein. CD20ALKT CD23, CD23, CD21, CD31, CD35, CD35, and CKL negative for CK5 / EB6 and CKL were negative in situ hybridization. Conclusion FRCT is rare in clinical practice, especially in tonsils, and it is more rare in tonsils, especially in tonsils, and the incidence of FRCT in tonsils is even rarer than that in tonsils, and the incidence of FRCT is more rare in tonsils than in tonsils. Diagnosis mainly depends on pathology and immunophenotype.
