【摘要】：目的探討鼻咽癌組織中組織蛋白酶D(Cathepsin D)和基質(zhì)金屬蛋白酶7(MMP-7)表達特點及其聯(lián)合評估腫瘤侵襲轉(zhuǎn)移的價值。方法采用免疫組化法檢測42例鼻咽癌活檢組織(A組)和30例同期慢性鼻咽炎活檢組織(B組)中Cathepsin D和MMP-7蛋白表達情況。比較鼻咽癌組織Cathepsin D和MMP-7蛋白陽性表達與其陰性表達患者的腫瘤T分期、N分期、微血管密度(MVD)及淋巴管密度(LVD)等。采用Spearman無條件相關(guān)分析法分析鼻咽癌組織Cathepsin D蛋白陽性表達率和MMP-7蛋白陽性表達率與其T分期、N分期、MVD及LVD的關(guān)系。結(jié)果與B組比較,A組Cathepsin D和MMP-7蛋白陽性表達率均提高(P0.05)。A組Cathepsin D蛋白陽性表達+MMP-7蛋白陽性表達患者的T3~4分期患者比例、N_(2~3)患者比例均高于其Cathepsin D蛋白陰性表達+MMP-7蛋白陽性表達患者和Cathepsin D蛋白陰性表達+MMP-7蛋白陰性表達患者(P0.05)。A組Cathepsin D蛋白陽性表達+MMP-7蛋白陽性表達患者的MVD及LVD均高于其Cathepsin D蛋白陽性表達+MMP-7蛋白陰性表達患者、Cathepsin D蛋白陰性表達+MMP-7蛋白陽性表達患者和Cathepsin D蛋白陰性表達+MMP-7蛋白陰性表達患者(P0.05)。A組Cathepsin D蛋白陽性表達+MMP-7蛋白陰性表達患者和Cathepsin D蛋白陰性表達+MMP-7蛋白陽性表達患者的MVD及LVD亦均高于其Cathepsin D蛋白陰性表達+MMP-7蛋白陰性表達患者(P0.05)。Spearman無條件相關(guān)分析結(jié)果顯示,鼻咽癌組織Cathepsin D蛋白陽性表達率和MMP-7蛋白陽性表達率與其T分期、N分期、MVD及LVD均呈正相關(guān)(r=0.798、0.785、0.855、0.842,P0.05)。結(jié)論鼻咽癌組織Cathepsin D和MMP-7蛋白陽性表達率高且均與其T分期、N分期、MVD及LVD相關(guān),聯(lián)合檢測鼻咽癌組織Cathepsin D和MMP-7蛋白表達可作為鼻咽癌腫瘤侵襲轉(zhuǎn)移的評估的參考指標。
[Abstract]:Objective to investigate the expression of cathepsin D in nasopharyngeal carcinoma (Cathepsin D) and matrix metalloproteinase 7 (MMP-7) expression and combined evaluation characteristics of tumor invasion and metastasis. Methods using immunohistochemistry in 42 cases of nasopharyngeal carcinoma biopsy (A group) and 30 cases of chronic nasopharyngitis biopsies (B group) in Cathepsin D and MMP-7 the expression of Cathepsin in nasopharyngeal carcinoma. The positive expression of D and MMP-7 protein and negative expression in patients with tumor T stage, N stage, microvessel density (MVD) and lymphatic vessel density (LVD). Using Spearman correlation analysis method to analyze the condition of nasopharyngeal carcinoma Cathepsin positive expression rate of D protein and MMP-7 protein positive expression rate with the T staging, N staging, the relationship between MVD and LVD. Results compared with B group, A group, Cathepsin D and MMP-7 protein expression rate was increased (P0.05) positive expression of.A Cathepsin D protein +MMP-7 protein positive group The expression of T3~4 in patients with stage N_ (2~3) the proportion of patients, the proportion of patients were higher than those of the Cathepsin negative expression of D protein positive expression of +MMP-7 protein and Cathepsin protein in patients with D negative expression of +MMP-7 protein in patients with negative expression of MVD (P0.05) and LVD.A group Cathepsin positive expression of D protein positive expression of +MMP-7 protein in patients with positive expression of Cathepsin was higher than that of the D +MMP-7 protein expression in patients with negative Cathepsin, negative expression of D protein positive expression of +MMP-7 protein and Cathepsin protein in patients with D negative expression of +MMP-7 protein in patients with negative expression of Cathepsin (P0.05).A group D protein positive expression of +MMP-7 protein in patients with negative expression of Cathepsin and D protein negative expression of MVD and LVD positive expression of +MMP-7 protein in patients were higher than those in the Cathepsin D protein negative expression of +MMP-7 protein in patients with negative expression (P0.05).Spearman unconditional correlation analysis showed that nasopharyngeal carcinoma group The Cathepsin positive expression rate of D protein and the expression rate of MMP-7 and T staging, N staging, MVD and LVD were positively correlated (r=0.798,0.785,0.855,0.842, P0.05). Conclusion Cathepsin in nasopharyngeal carcinoma D and MMP-7 protein positive expression rate is high and the T stage, N stage, MVD and LVD, the reference index of combined detection of nasopharyngeal carcinoma Cathepsin D and MMP-7 protein expression in nasopharyngeal carcinoma can be used as evaluation of tumor invasion and metastasis.
