【摘要】：目的觀察鞏膜外硅膠海綿墊壓術(shù)和鞏膜外環(huán)扎聯(lián)合硅膠墊壓術(shù)后眼球結(jié)構(gòu)的改變及對(duì)眼壓的影響。設(shè)計(jì)回顧性病例系列。研究對(duì)象孔源性視網(wǎng)膜脫離住院手術(shù)治療患者64例64眼,其中接受鞏膜外單純硅膠海綿墊壓術(shù)25眼(海綿組),鞏膜外環(huán)扎聯(lián)合硅膠墊壓術(shù)39眼(聯(lián)合組)。方法手術(shù)前,術(shù)后1周、1個(gè)月及3個(gè)月應(yīng)用A型超聲儀、超聲生物顯微鏡(UBM)對(duì)眼球生物參數(shù)檢測(cè),非接觸眼壓計(jì)檢測(cè)眼壓。主要指標(biāo)眼壓、角膜厚度、角膜曲率、前房深度、晶狀體厚度、睫狀體厚度、房角開(kāi)放距離和小梁虹膜夾角。結(jié)果術(shù)后3個(gè)月時(shí)64眼全部視網(wǎng)膜解剖復(fù)位。眼壓、角膜厚度、角膜水平曲率、角膜垂直曲率、晶狀體厚度、睫狀體厚度,術(shù)后1周與術(shù)前相比顯著增加(P均0.05)。海綿組術(shù)后1個(gè)月和3個(gè)月眼壓與術(shù)前相比無(wú)顯著性差異,而聯(lián)合組術(shù)后3個(gè)月眼壓(15.41±2.58)mmHg與術(shù)前(13.66±3.16)mmHg相比有顯著性差異(P=0.009)。海綿組和聯(lián)合組角膜垂直曲率術(shù)后1周分別為(45.03±1.08)D、(44.47±0.90)D(P=0.027)。結(jié)論鞏膜外硅膠海綿墊壓術(shù)和鞏膜外環(huán)扎聯(lián)合硅膠墊壓術(shù)對(duì)角膜、前房及房角等眼球結(jié)構(gòu)的早期影響較明顯;鞏膜外環(huán)扎聯(lián)合硅膠墊壓術(shù)對(duì)眼壓影響持續(xù)時(shí)間長(zhǎng),應(yīng)給予關(guān)注和干預(yù)。
[Abstract]:Objective to observe the changes of ocular structure and the effect on intraocular pressure after scleral silicone sponge pad compression and scleral external ligation combined with silicone pad pressure. Design of retrospective series of cases. 64 patients (64 eyes), Among them, 25 eyes were treated with simple silicone sponge pad pressing (sponge group), 39 eyes were treated with external scleral ring ligation and silicone pad pressing (combined group). Methods A type A ultrasound instrument was used before operation, 1 week, 1 month and 3 months after operation. Ultrasound biomicroscopy (UBM) for the detection of ocular biological parameters, non-contact intraocular pressure (NIOP) for intraocular pressure. Main outcome measures: intraocular pressure, corneal thickness, corneal curvature, anterior chamber depth, lens thickness, ciliary body thickness, Results at 3 months after operation, all 64 eyes had retinal reattachment, intraocular pressure, corneal thickness, corneal horizontal curvature, corneal vertical curvature, lens thickness, ciliary body thickness. There was no significant difference in IOP between the sponge group and the preoperative group at 1 and 3 months after operation, and there was no significant difference between the sponge group and the preoperative group. The IOP of the combined group was 15.41 鹵2.58 mm Hg and 13.66 鹵3.16 mm Hg respectively 3 months after operation. The corneal vertical curvature in the sponge group and the combined group was 45.03 鹵1.08D, 44.47 鹵0.90DP0.0277.Conclusion the cornea of the combined group were treated by external scleral silicone sponge pad compression and scleral ring ligation combined with silicone pad compression. The early influence of anterior chamber and angle of eyeball was obvious, and the effect of scleral ligation combined with silicone pad pressure on intraocular pressure lasted for a long time, so attention and intervention should be paid to it.
