【摘要】：目的:探討先天性聽骨鏈畸形的診斷及手術(shù)療效。方法:14例(15耳)先天性聽骨鏈畸形患者,其中10例為單側(cè)傳導(dǎo)性聾,1例為雙側(cè)傳導(dǎo)性聾,3例混合性聾且以傳導(dǎo)性為主。15耳均行鼓室探查,并行14耳聽骨鏈重建術(shù)(3耳行全聽骨重建術(shù),10耳行部分聽骨重建術(shù),1耳行人工鐙骨手術(shù)),1例松解聽骨鏈與鼓室壁的軟骨固定。通過術(shù)前術(shù)后氣骨導(dǎo)差比較療效。結(jié)果:術(shù)前聽力檢查示平均氣骨導(dǎo)差為39.9dB,術(shù)后3周平均氣骨導(dǎo)差為22.3dB,較術(shù)前縮小17.6dB(t=22.10,P0.01),術(shù)后6~8個月平均氣骨導(dǎo)差為19.6dB,較術(shù)前縮小20.3dB(t=12.813,P0.01)。結(jié)論:先天性聽骨鏈畸形引起的傳導(dǎo)性聽力下降,根據(jù)術(shù)中情況采用不同聽骨鏈重建術(shù)可以明顯提高聽力,以手術(shù)治療為首選。
[Abstract]:Objective: to investigate the diagnosis and operative effect of congenital ossicular chain malformation. Among them, 10 cases were unilateral conductive deafness, 1 case was bilateral conductive deafness and 3 cases were mixed deafness. Total ossicular reconstruction was performed in 10 ears and partial ossicular reconstruction was performed in 10 ears. Artificial stapes reconstruction was performed in 1 case of cartilage fixation of auditory chain and tympanic wall in 1 case. The results were compared by air bone conductance difference before and after operation. Results: the average air-bone conductance difference was 39.9 dB before operation and 22.3dBat 3 weeks after operation, which was reduced by 17.6dB, 22.10dP0.01g, 19.6dBand 20.3dBt12.813P0.01a, respectively. Conclusion: congenital ossicular chain deformity can cause the decrease of conductive hearing. Different ossicular chain reconstruction can significantly improve the hearing according to the intraoperative conditions, and surgical treatment is the first choice.
