【摘要】：目的通過(guò)高危兔眼模型分析角膜后彈力層撕除角膜內(nèi)皮移植術(shù)后效果及免疫排斥反應(yīng)特點(diǎn)。方法根據(jù)手術(shù)方法的不同將28只兔分成2組:穿透性角膜移植組(PK組)和角膜后彈力層撕除角膜內(nèi)皮移植組(DSEK組),每組各14只,右眼為手術(shù)眼。術(shù)后用裂隙燈檢查角膜混濁、水腫和新生血管情況,并記錄排斥指數(shù)(rejection index,RI)和新生血管指數(shù)(neovascularization index,NI)。利用HE染色進(jìn)行病理分析。采用實(shí)時(shí)熒光定量PCR技術(shù)測(cè)定各組白細(xì)胞介素-2(interleukin-2,IL-2)、IL-10和血管內(nèi)皮生長(zhǎng)因子(vas-cular endothelial growth factor,VEGF)mR NA的表達(dá)。結(jié)果眼前節(jié)檢查顯示PK組大部分兔眼角膜水腫、混濁及大量新生血管,而DSEK組只有少量兔眼出現(xiàn)角膜混濁水腫。PK組13眼(92.9%)出現(xiàn)排斥反應(yīng),明顯高于DSEK組的4眼(28.6%),差異有統(tǒng)計(jì)學(xué)意義(P0.05)。病理組織切片顯示,PK組單核細(xì)胞浸潤(rùn)明顯高于DSEK組。實(shí)時(shí)熒光定量PCR檢測(cè)結(jié)果顯示,PK組IL-2、IL-10和VEGF mR NA的表達(dá)均明顯高于DSEK組。結(jié)論 DSEK術(shù)后出現(xiàn)免疫排斥反應(yīng)幾率低,且比PK術(shù)后排斥反應(yīng)程度弱,角膜內(nèi)皮移植術(shù)是治療角膜內(nèi)皮疾病的一個(gè)理想選擇。
[Abstract]:Objective to analyze the effect of corneal posterior elastic layer avulsion after corneal endothelium transplantation and the characteristics of immune rejection in high-risk rabbit eyes. Methods 28 rabbits were divided into two groups according to the different surgical methods: penetrating keratoplasty group and PK group. ) and DSEK group (14 rats in each group). The right eye was operated on. Corneal opacity, edema and neovascularization were examined by slit lamp after operation. The rejection index (RI) and neovascularization index (neovascularization) were recorded. The expression of interleukin-2interleukin-2IL-2mR10 and vascular endothelial growth factor (VEGF) were measured by real-time fluorescence quantitative PCR. The corneal edema of most of the rabbits in the competition group was revealed by the examination of the anterior segment of the fruit. Opacity and neovascularization, while in DSEK group, only a few rabbit eyes had corneal opacity edema. In PK group, 13 eyes (92.9 eyes) showed rejection. The expression of IL-2IL-10 and VEGF mRNA in the PK group was significantly higher than that in the DSEK group, and the expression of IL-2 IL-10 and VEGF mRNA in the PK group was significantly higher than that in the DSEK group. Conclusion the incidence of immune rejection after DSEK is low. Corneal endothelial transplantation is an ideal choice for the treatment of corneal endothelial diseases.
