【摘要】：目的探討術(shù)前全身化療對(duì)眼內(nèi)期視網(wǎng)膜母細(xì)胞瘤(RB)術(shù)后眼球病理分級(jí)及生存率的影響。方法選取北京同仁醫(yī)院兒科2008年1月-2009年12月術(shù)前臨床診斷同時(shí)經(jīng)眼球摘除后病理確診的為D期及E期的223例RB患兒為研究對(duì)象。根據(jù)其病理分級(jí)標(biāo)準(zhǔn)腫瘤侵犯視神經(jīng)的程度分為5級(jí):0級(jí)為未侵犯視神經(jīng);1級(jí)為侵犯視乳頭;2級(jí)為侵犯篩板;3級(jí)為侵犯篩板之后,未到視神經(jīng)斷端;4級(jí)為侵犯視神經(jīng)斷端。分別將D、E期患兒根據(jù)術(shù)前化療與否分為2組:第一組術(shù)前未行化療組;第二組術(shù)前化療組(1~6個(gè)周期)。分析兩組眼病理視神經(jīng)侵犯程度及預(yù)后。結(jié)果 223例患兒共263只眼,其中D期68只眼,E期患兒155只眼,雙眼中對(duì)側(cè)眼A期28只眼,B期12只眼;雙眼患兒40例,單眼183例;男148例,女95例;發(fā)病平均年齡24.4±17.0個(gè)月,中位發(fā)病年齡21個(gè)月。D期及E期患兒術(shù)前未行化療組與化療組眼病理視神經(jīng)侵犯程度差異均具有顯著性;D期患兒術(shù)前未行化療組與化療組眼病理腫瘤細(xì)胞穿過(guò)篩板發(fā)生率差異無(wú)顯著性(P0.05),而E期術(shù)前未行化療組與化療組眼病理腫瘤細(xì)胞穿過(guò)篩板發(fā)生率差異具有顯著性(P0.05)。術(shù)后5年生存率D期為98.5%,E期為96.1%。D期及E期術(shù)前化療組與未化療組之間生存率差異無(wú)顯著性(P0.05)。結(jié)論術(shù)前全身化療是眼內(nèi)期分期D、E期RB的術(shù)后病理分級(jí)的影響因素。對(duì)D、E期RB患兒術(shù)前行化療與否并不影響患兒的總生存率。
[Abstract]:Objective to explore the effect of preoperative chemotherapy on intraocular intraocular retinoblastoma (RB) postoperative eyeball pathological grading and survival. Methods from January 2008 -2009 December Beijing Tongren Hospital pediatric preoperative clinical diagnosis and pathology diagnosis by enucleation in 223 cases of children with RB D and E as the research object.
