【摘要】：[目的]比較術(shù)前放療加手術(shù)與單純手術(shù)治療喉癌的療效,為喉癌治療方式的選擇提供可靠的循證依據(jù)。[方法]檢索國內(nèi)外有關(guān)數(shù)據(jù)庫查找符合檢索條件的前瞻性臨床隨機對照試驗,采用Meta分析的方法,對國內(nèi)外公開發(fā)表的有關(guān)術(shù)前放療加手術(shù)與單純手術(shù)治療喉癌的臨床隨機對照試驗的研究文獻進行綜合分析。[結(jié)果]共納入5個隨機對照試驗,研究對象共1086例。術(shù)前放療加手術(shù)與單純手術(shù)相比并未提高喉癌患者的3和5年生存率,RR分別為0.94(95%CI為0.71~1.24)和1.09(95%CI為0.88~1.34)。術(shù)前放療加手術(shù)與單純手術(shù)相比治療聲門上型、聲門型以及Ⅱ、Ⅲ、Ⅳ期喉癌3年生存率,差異均無統(tǒng)計學意義,RR分別為0.93(95%CI為0.61~1.42,P=0.73)、0.55(95%CI為0.26~1.19,P=0.13)、0.86(95%CI為0.34~2.15,P=0.74)、0.82(95%CI為0.46~1.47,P=0.50)和0.74(95%CI為0.44~1.25,P=0.26)。術(shù)前放療加手術(shù)與單純手術(shù)相比治療聲門上型、聲門型以及Ⅰ/Ⅱ和Ⅲ/Ⅳ期喉癌5年生存率,差異均無統(tǒng)計學意義,RR分別為1.01(95%CI為0.80~1.28,P=0.92)、1.06(95%CI為0.67~1.67,P=0.80)、0.97(95%CI為0.86~1.09,P=0.63)和1.01(95%CI為0.90~1.13,P=0.91)。[結(jié)論 ]本系統(tǒng)評價納入研究有限,結(jié)果提示術(shù)前放療加手術(shù)治療對比手術(shù)治療并未提高喉癌患者的生存率,需進一步開展更多高質(zhì)量臨床研究進行驗證。
[Abstract]:[Objective] to compare the curative effect of preoperative radiotherapy plus operation on laryngeal carcinoma and to provide reliable evidence-based basis for the choice of treatment mode of laryngeal carcinoma. [Methods] A prospective randomized controlled trial was used to search the relevant databases at home and abroad, which met the search criteria. The method of Meta analysis was used. The literature published at home and abroad on clinical randomized controlled trials of preoperative radiotherapy plus surgery and simple surgery for laryngeal carcinoma was analyzed. [Results] A total of 1 086 patients were enrolled in 5 randomized controlled trials. Preoperative radiotherapy plus surgery did not improve the 3 and 5 year survival rate of laryngeal cancer patients compared with surgery alone.
