【摘要】：目的:總結(jié)分析顳骨類癌的臨床特征、鑒別診斷及手術(shù)治療方法。方法:回顧性總結(jié)分析6例經(jīng)病理證實(shí)的顳骨類癌患者的臨床資料,并復(fù)習(xí)相關(guān)文獻(xiàn)。依據(jù)病變范圍行顳骨次全切除手術(shù)或顳骨擴(kuò)大切除術(shù)。結(jié)果:6例患者影像學(xué)資料示顳骨不同范圍的占位病變,術(shù)后病理均為顳骨類癌,最終診斷為單側(cè)原發(fā)性顳骨類癌。第1次手術(shù)后,面癱復(fù)發(fā)2例,顱內(nèi)轉(zhuǎn)移1例。治療后無并發(fā)癥,隨訪5例無復(fù)發(fā)及遠(yuǎn)處轉(zhuǎn)移,僅1例因肝轉(zhuǎn)移死亡。結(jié)論:顳骨類癌臨床上少見,無特征性臨床及影像學(xué)表現(xiàn),易與中耳炎或中耳其他良、惡性腫瘤混淆。確診主要依靠病理及免疫組織化學(xué)。一般以手術(shù)治療為主。早期發(fā)現(xiàn)并徹底切除腫瘤者轉(zhuǎn)移和復(fù)發(fā)率低。
[Abstract]:Objective: To summarize and analyze the clinical features, differential diagnosis and surgical treatment of temporal bone like carcinoma. Methods: the clinical data of 6 patients with the pathologically confirmed temporal bone carcinoid were reviewed and analyzed, and the related literature was reviewed. Subtotal resection of the temporal bone or enlarged excision of temporal bone was performed on the basis of the lesion range. Results: the imaging data of 6 cases showed the occupying lesion of the temporal bone in different ranges, and the postoperative pathology was the type of the temporal bone carcinoid, and the final diagnosis was the primary primary temporal bone carcinoid. After the first operation, there were 2 cases of facial paralysis and 1 cases of intracranial metastasis. There were no complications after treatment. There were no recurrence and distant metastasis in 5 cases. Only 1 cases died of liver metastasis. Conclusion: the temporal bone carcinoid is rare in clinical, without characteristic clinical and imaging findings, and is easily confused with otitis media or other benign and malignant tumors of the middle ear. The diagnosis mainly depends on the pathology and immunohistochemistry. Generally, surgical treatment is the main treatment. The rate of metastasis and recurrence was low in the early detection and total resection of the tumor.
