文章摘要：目的通過(guò)研究發(fā)現(xiàn)影響泌尿外科老年患者下肢活動(dòng)依從性的因素，制訂切實(shí)可行的下肢活動(dòng)方案，預(yù)防患者術(shù)后下肢靜脈血栓的形成。方法在泌尿外科病房選取研究者所在責(zé)任組內(nèi)行手術(shù)治療患者共22例進(jìn)行下肢活動(dòng)指導(dǎo)，，根據(jù)Lewin提出的行動(dòng)研究框架，以計(jì)劃、行動(dòng)、觀察、反思4個(gè)步驟進(jìn)行螺旋式循環(huán)，評(píng)價(jià)活動(dòng)方案的有效性和可行性，采用詢問(wèn)法評(píng)估患者的依從性。結(jié)果實(shí)踐10周，共完成4輪循環(huán)。經(jīng)過(guò)實(shí)踐細(xì)化護(hù)理程序，完善危險(xiǎn)因素評(píng)估表，調(diào)整宣教時(shí)間等，使被干預(yù)患者對(duì)下肢活動(dòng)重要性的認(rèn)知程度和接受程度得到了提高，活動(dòng)的依從性良好。結(jié)論本研究所制訂的下肢活動(dòng)方案經(jīng)過(guò)行動(dòng)研究實(shí)踐的修改及完善。能夠提高患者下肢活動(dòng)的依從性，以預(yù)防深靜脈血栓的發(fā)生。

Abstr：Objective To explore the influential factors of compliance of lower limbs exercise,and to develop efficient program of lower limbs exercise to prevent deep venous thrombosis（DVT） for elderly patients after urinary surgery. Methods Twenty-two elderly patients after urinary surgery were recruited in this study. The patients were asked to do lower limbs exercises guided by the nurses. Based on the framework of Lewin＇s action research, researchers evaluated the effectiveness and feasibility of the lower limbs exercise program with the cycles of diagnosing, planning, action taking,observing and reconsidering. In addition, the compliance of patients was assessed through interview. Results After ten weeks of practice, 4 cycles has been finished. In this period,the researchers improved the evaluation form of dangerous factors,refined the nursing process and adjusted the education schedule, which improved the patients＇ recognition and acceptance to the importance of lower limbs exercise, and promoted the compliance of lower limbs exercise. Conclusion The lower limbs exercise program has been revised and perfected through action research and has been proved to be effective to prevent the incidence of DVT.
