體力狀況及營養(yǎng)風險對胰十二指腸切除術后并發(fā)癥的影響

目的:探討術前體力狀況和營養(yǎng)風險的相關性及對胰十二指腸切除術(PD)后并發(fā)癥的影響,初步評價兩者對術后并發(fā)癥的預測價值。方法:回顧性分析浙江大學醫(yī)學院附屬邵逸夫醫(yī)院肝膽胰外科于2013年1月至2016年9月收治入院的125例行胰十二指腸切除術(PD)患者的臨床資料,術前根據(jù)體力狀況評分(ZPS,5分法)和NRS 2002對PD患者進行體力狀況和營養(yǎng)風險的評估。臨床結局設定為術后并發(fā)癥的發(fā)生,并按術后并發(fā)癥Clavien系統(tǒng)分級將并發(fā)癥分為嚴重并發(fā)癥(Clavien分級Ⅲ、Ⅳ、Ⅴ級)和一般并發(fā)癥(Clavien分級Ⅰ、Ⅱ級)。采用Spearman秩相關性系數(shù)表示術前體力狀況評分和NRS2002的相關性,通過X2檢驗分析兩者與術后并發(fā)癥之間的關系;并對術前可能與并發(fā)癥相關的因素行單因素分析及非條件Logistic回歸分析。結果:1.125例PD術后總的并發(fā)癥53例,發(fā)病率為42.4%。其中嚴重并發(fā)癥18例(24例次),發(fā)病率為17.6%,死亡4例(3.2%)。2.術前NRS2002≥3即存在營養(yǎng)風險者61例(48.8%),ZPS≥2即體力狀況不佳者32例(25.6%),體力狀況評分與NRS...
