【摘要】：目的 :評估分析彎角椎體成形術(shù)(percutaneous curved vertebroplasty,PCVP)治療胸腰椎骨質(zhì)疏松性椎體壓縮骨折的臨床療效。方法:選取96例胸腰椎骨質(zhì)疏松性椎體壓縮骨折患者,共128節(jié)病椎,將其分為彎角組(36例,43節(jié))、單側(cè)組(32例,42節(jié))及雙側(cè)組(28例,43節(jié)),分別行PCVP及單側(cè)、雙側(cè)椎弓根入路椎體成形術(shù)。3組患者的性別、年齡、骨密度及病椎節(jié)段分布均無統(tǒng)計學(xué)差異(P0.05)。統(tǒng)計3組手術(shù)時間、X線曝光次數(shù)、骨水泥注入量及術(shù)前、術(shù)后24h、術(shù)后3個月疼痛視覺模擬評分(VAS評分),術(shù)后復(fù)查CT評價骨水泥滲漏率,進(jìn)行統(tǒng)計學(xué)分析。結(jié)果:彎角組與單側(cè)組的手術(shù)時間、X線曝光次數(shù)均明顯少于雙側(cè)組,差異有統(tǒng)計學(xué)意義(P0.05),彎角組和單側(cè)組無顯著性差異(P0.05)。彎角組、單側(cè)組、雙側(cè)組的骨水泥注入量比較,雙側(cè)組最多(6.2±1.5ml),彎角組次之(4.5±1.3ml),單側(cè)組最少(3.4±1.2ml),組間兩兩比較差異有統(tǒng)計學(xué)意義(P0.05)。彎角組、單側(cè)組、雙側(cè)組的骨水泥滲漏率比較,單側(cè)組(28.6%,12/42)最高,雙側(cè)組(18.6%,8/43)次之,彎角組(9.3%,4/43)最低,組間兩兩比較差異有統(tǒng)計學(xué)意義(P0.05)。3組患者術(shù)后疼痛均明顯緩解,術(shù)后24h及術(shù)后3個月VAS評分均明顯低于術(shù)前(P0.05),術(shù)后24h及術(shù)后3個月VAS評分比較無顯著性差異(P0.05),同時間點組間比較亦無顯著性差異(P0.05)。結(jié)論:PCVP具有操作簡單、手術(shù)時間短、X線透視次數(shù)少、創(chuàng)傷小、并發(fā)癥少等優(yōu)勢,是治療胸腰椎骨質(zhì)疏松性椎體壓縮骨折的有效微創(chuàng)手術(shù)方法。
[Abstract]:Objective: to evaluate the clinical effect of angle vertebroplasty (percutaneous curved vertebroplasty,PCVP) in the treatment of osteoporotic vertebral compression fracture of thoracolumbar vertebrae. Methods: 96 patients with thoracolumbar osteoporotic vertebral compression fracture were divided into angle group (n = 36, n = 43), unilateral group (n = 32, 42 nodes) and bilateral group (n = 28, 43 nodes). There were no significant differences in sex, age, bone mineral density and distribution of diseased vertebrae between the three groups (P 0.05). The operation time, X-ray exposure times, bone cement injection volume and visual simulation score (VAS score) of pain 24 hours and 3 months after operation were counted. The leakage rate of bone cement was evaluated by CT after operation, and statistical analysis was carried out. Results: the operation time and X-ray exposure times of angle bending group and unilateral group were significantly lower than those of bilateral group, the difference was statistically significant (P 0.05), but there was no significant difference between angle bending group and unilateral group (P 0.05). The bone cement injection in the angle group, unilateral group and bilateral group was the most (6.2 鹵1.5 ml), followed by the angle group (4.5 鹵1.3 ml) and the unilateral group (3.4 鹵1.2 ml).
