【摘要】：目的探討全胸腔鏡下肺段切除術(shù)的臨床應用價值。方法回顧性分析2013年1月~2016年5月41例全胸腔鏡下肺段切除術(shù)的臨床資料,其中淮南市第一人民醫(yī)院12例,安徽省立醫(yī)院29例。三孔法,肺段動脈、靜脈使用Hem-olok或直線型切割縫合器處理,肺段支氣管及段間平面均使用直線型切割縫合器處理。結(jié)果 41例手術(shù)均順利完成,無中轉(zhuǎn)開胸及輔助小切口。手術(shù)時間89~208(121.0±15.5)min,術(shù)中出血量80~350(142.7±11.5)ml。21例原發(fā)肺癌行肺門縱隔淋巴結(jié)清掃,清掃淋巴結(jié)7~15(10.9±0.6)枚。無圍手術(shù)期死亡。二次手術(shù)2例(1例術(shù)后咯血,1例肺不張)。術(shù)后胸腔引流時間5~9(7.8±0.2)d,術(shù)后住院時間6~14(9.3±0.4)d。術(shù)后病理診斷腺癌16例,鱗癌5例,轉(zhuǎn)移癌3例,支氣管擴張8例,炎性假瘤4例,結(jié)核球4例,血管畸形1例。對21例原發(fā)肺癌術(shù)后隨訪2~35(25.4±8.8)月,無復發(fā)、轉(zhuǎn)移。結(jié)論全胸腔鏡下解剖性肺段切除術(shù)安全、可行、有效。
[Abstract]:Objective to evaluate the clinical value of total thoracoscopic pulmonary segmental resection. Methods the clinical data of 41 cases of total thoracoscopic segmental pulmonary resection from January 2013 to May 2016 were analyzed retrospectively, including 12 cases in Huainan first people's Hospital and 29 cases in Anhui Provincial Hospital. Three-hole method, pulmonary segmental artery, vein were treated with Hem-olok or linear cutting suture, pulmonary segmental bronchus and intersegmental plane were treated with linear cutting suture. Results all the 41 cases were successfully completed without conversion to thoracotomy and auxiliary small incision. The operation time was 89 鈮，
