【摘要】：目的探討使用改良縫合法治療急性閉合性跟腱斷裂的臨床療效。方法 2013年12月—2016年12月,采用改良縫合法治療29例急性閉合性跟腱斷裂。男23例,女6例;年齡22~45歲,平均34歲�；颊呔鶠檫\(yùn)動損傷,其中左側(cè)22例、右側(cè)7例。術(shù)前美國足踝外科協(xié)會(AOFAS)踝與后足功能評分為(44.6±3.6)分;參照Amer-Lindholm療效評定標(biāo)準(zhǔn)評價均為差。MRI檢查顯示跟腱完全斷裂。結(jié)果手術(shù)時間35~62 min,平均46 min;術(shù)中出血量7~15 m L,平均10 m L。術(shù)后切口均Ⅰ期愈合,無腓腸神經(jīng)損傷發(fā)生。患者均獲隨訪,隨訪時間5~38個月,平均18.4個月。術(shù)后3個月,AOFAS踝與后足功能評分為(93.1±4.3)分,與術(shù)前比較差異有統(tǒng)計(jì)學(xué)意義(t= 49.581,P=0.000)。參照Amer-Lindholm療效評定標(biāo)準(zhǔn)評價,獲優(yōu)24例、良5例,優(yōu)良率100%;與術(shù)前比較差異有統(tǒng)計(jì)學(xué)意義(Z= 7.294,P=0.000)。MRI檢查示跟腱連續(xù)性完整。隨訪期間均無跟腱再斷裂發(fā)生。結(jié)論改良縫合法治療急性閉合性跟腱斷裂具有創(chuàng)傷小、再斷裂發(fā)生率低、腓腸神經(jīng)損傷風(fēng)險低及功能恢復(fù)較好等優(yōu)點(diǎn)。
[Abstract]:Objective to investigate the clinical effect of modified suture in the treatment of acute closed Achilles tendon rupture. Methods from December 2013 to December 2016, 29 cases of acute closed Achilles tendon rupture were treated with modified suture. There were 23 males and 6 females with an average age of 34 years. All the patients had motor injury, including 22 cases on the left side and 7 cases on the right side. The score of ankle and posterior foot function of the American Association of foot and ankle surgery was (44.6 鹵3.6) before operation, and the evaluation was poor according to the evaluation standard of Amer-Lindholm. MRI showed complete rupture of Achilles tendon. Results the operation time was 35 鈮，
