【摘要】：目的觀察老年股骨粗隆間粉碎骨折患者實(shí)施帶股骨距假體置換治療的臨床效果。方法將84例老年股骨粗隆間粉碎骨折患者作為受試對(duì)象,利用隨機(jī)數(shù)字表分為觀察組(n=42)和對(duì)照組(n=42)。對(duì)照組實(shí)施常規(guī)手術(shù)治療,觀察組實(shí)施帶股骨距假體置換治療。觀察兩組手術(shù)持續(xù)時(shí)間、術(shù)中出血量、術(shù)后下地活動(dòng)時(shí)間和住院時(shí)間,對(duì)比兩組治療前后關(guān)節(jié)功能評(píng)分(Harris量表)及并發(fā)癥發(fā)生情況。結(jié)果觀察組手術(shù)持續(xù)時(shí)間、術(shù)后下地活動(dòng)時(shí)間和住院時(shí)間均較對(duì)照組明顯縮短(P0.05),且觀察組術(shù)中出血量顯著少于對(duì)照組(P0.05);治療前觀察組與對(duì)照組各維度Harris評(píng)分無(wú)顯著差異(P0.05),治療后兩組評(píng)分均較治療前顯著升高(P0.05),且觀察組評(píng)分均遠(yuǎn)高于對(duì)照組(P0.05);兩組并發(fā)癥發(fā)生率比較有顯著差異(P0.05)。結(jié)論對(duì)老年股骨粗隆間粉碎骨折實(shí)施帶股骨距假體置換治療能夠有效減輕創(chuàng)傷,還可改善關(guān)節(jié)功能。
[Abstract]:Objective to observe the clinical effect of femoral intertrochanteric comminuted fracture in elderly patients with femoral intertrochanteric comminuted fracture. Methods 84 elderly patients with comminuted femoral intertrochanteric fracture were randomly divided into observation group (n = 42) and control group (n = 42). The patients in the control group were treated with routine operation and the patients in the observation group were treated with femoral talus prosthesis replacement. The duration of operation, the amount of bleeding during the operation, the time of postoperative activity and the length of hospitalization were observed. The joint function scale (Harris) and the occurrence of complications were compared between the two groups before and after treatment. Results compared with the control group, the duration of operation, the postoperative activity time and the hospitalization time in the observation group were significantly shorter than those in the control group (P 0.05), and the amount of bleeding in the observation group was significantly less than that in the control group (P 0.05). There was no significant difference in Harris scores between the two groups before and after treatment (P0.05). After treatment, the scores in the two groups were significantly higher than those before treatment (P0.05), and the scores in the observation group were significantly higher than those in the control group (P0.05). There was a significant difference in the incidence of complications between the two groups (P0.05). Conclusion replacement of femoral intertrochanteric comminuted fracture in elderly patients with femoral intertrochanteric comminuted fracture can effectively reduce trauma and improve joint function.
