【摘要】：目的:觀察探討多軸鎖定鋼板結(jié)合關(guān)節(jié)鏡技術(shù)治療高能量Pilon骨折的療效。方法:自2010-02至2013-04,解放軍100醫(yī)院骨科手術(shù)治療高能量Pilon骨折22例,術(shù)前嚴(yán)密設(shè)計(jì),利用CT三維掃描重建骨折形態(tài)及關(guān)節(jié)面情況,術(shù)中采用關(guān)節(jié)鏡技術(shù)輔助骨折復(fù)位及固定,利用多軸鎖定鋼板對(duì)脛骨遠(yuǎn)端粉碎性骨折進(jìn)行固定,術(shù)后規(guī)范康復(fù)訓(xùn)練。通過分析手術(shù)時(shí)間、手術(shù)出血量及末次隨訪時(shí)踝關(guān)節(jié)的功能評(píng)分(Tornetta評(píng)價(jià)和Mazur評(píng)分)、影像學(xué)評(píng)價(jià)和術(shù)后并發(fā)癥的發(fā)生情況評(píng)價(jià)多軸鎖定鋼板結(jié)合關(guān)節(jié)鏡技術(shù)治療高能量Pilon骨折的療效。結(jié)果:22例均獲得隨訪10~24個(gè)月,平均13.3個(gè)月。骨折均獲得愈合,愈合時(shí)間7~19周,平均11.7周。其中1例出現(xiàn)皮膚壞死,行轉(zhuǎn)移皮瓣修復(fù),其余無深部感染、骨折不愈合、鋼板外露等并發(fā)癥發(fā)生。末次隨訪時(shí)均行踝關(guān)節(jié)CT檢查,僅1例CT顯示脛骨遠(yuǎn)端關(guān)節(jié)面欠平整,但暫無骨性關(guān)節(jié)炎表現(xiàn);術(shù)后關(guān)節(jié)功能根據(jù)踝關(guān)節(jié)Tornetta評(píng)價(jià):優(yōu)16例,良4例,可2例,優(yōu)良率90.91%;根據(jù)踝關(guān)節(jié)Mazur評(píng)分評(píng)定:優(yōu)15例,良4例,可3例,優(yōu)良率86.4%。結(jié)論:采用多軸鎖定鋼板結(jié)合關(guān)節(jié)鏡技術(shù)治療高能量Pilon骨折療效值得肯定,有利于踝關(guān)節(jié)軟組織的保護(hù)及關(guān)節(jié)面的解剖修復(fù),有助于降低骨折早期及晚期的并發(fā)癥發(fā)生率。對(duì)比其他術(shù)式,采用多軸鎖定鋼板結(jié)合關(guān)節(jié)鏡技術(shù)治療高能量Pilon骨折手術(shù)創(chuàng)傷更小、固定更可靠、內(nèi)固定物的使用更少,是一種值得采用和推廣的手術(shù)方式。
[Abstract]:Objective: to investigate the efficacy of multiaxial locking plate combined with arthroscopy in the treatment of high energy Pilon fracture. Methods: from 2010 / 02 to 2013 / 04, 22 cases of high energy Pilon fractures were treated surgically in the Department of Orthopaedics of PLA 100 Hospital. The fracture morphology and articular surface were reconstructed by CT 3D scanning. Arthroscopic technique was used to assist in reduction and fixation of fractures, multiaxial locking plate was used to fix distal tibial comminuted fractures, and postoperative rehabilitation training was standardized. The time of operation, the amount of bleeding during operation and the score of ankle function at the last follow-up were analyzed (Tornetta and Mazur). Evaluation of imaging and postoperative complications to evaluate the efficacy of multiaxial locking plate combined with arthroscopy in the treatment of high energy Pilon fractures. Results: all 22 cases were followed up for 10 months for 24 months (mean 13.3 months). All the fractures were healed, the healing time was 7-19 weeks (mean 11.7 weeks). One case had skin necrosis, and the other had no complications such as deep infection, nonunion of fracture and exposure of steel plate. At the last follow-up, CT was performed on ankle joint, only 1 case showed that the distal tibial joint face was not smooth, but there was no osteoarthritis at the moment, the postoperative joint function was evaluated according to the Tornetta of ankle joint: excellent in 16 cases, good in 4 cases, fair in 2 cases, excellent and good rate was 90.91%; According to the Mazur score of ankle joint, 15 cases were excellent, 4 cases were good, 3 cases were fair, the excellent and good rate was 86.4%. Conclusion: the treatment of high energy Pilon fracture with multiaxial locking plate combined with arthroscopy is worth affirming. It is beneficial to the protection of soft tissue of ankle joint and anatomical repair of articular surface. It is helpful to reduce the incidence of complications in early and late stage of fracture. Compared with other surgical methods, multiaxial locking plate combined with arthroscopy for the treatment of high-energy Pilon fracture surgery less trauma, more reliable fixation, less use of internal fixation, it is worth to adopt and promote the operation.
