【摘要】：隨著我國(guó)逐漸步入老齡化社會(huì),退行性膝關(guān)節(jié)炎發(fā)病率正在逐年增高,而其中又以?xún)?nèi)翻型較為多見(jiàn)。由于膝關(guān)節(jié)骨性關(guān)節(jié)炎(KOA)病情發(fā)展隱匿,病因及發(fā)病機(jī)制研究仍不十分清楚,因此針對(duì)該病的早期診斷及治療顯得尤為重要。詳盡的查體、完善的影像學(xué)檢查為早期診斷KOA提供了理論依據(jù),也催發(fā)了針對(duì)該病的多種早期診療方案。由于目前各學(xué)科發(fā)展水平的不同,以及人們對(duì)KOA嚴(yán)重程度認(rèn)識(shí)的混亂,使得該病治療的矛盾點(diǎn)逐漸由延誤治療轉(zhuǎn)向?yàn)檫^(guò)度診療。如何根據(jù)病情進(jìn)展的不同階段,制定一套針對(duì)性強(qiáng)、行之有效的治療方案,避免延誤治療及過(guò)度診療的發(fā)生,成為了一項(xiàng)新的課題。本文就目前臨床常見(jiàn)的診療方案進(jìn)行逐一介紹,分述各適應(yīng)癥及優(yōu)缺點(diǎn),并著重體現(xiàn)各方案在不同KOA發(fā)病階段治療上的優(yōu)勢(shì)所在。
[Abstract]:With our country gradually entering the aging society, the incidence of degenerative knee arthritis is increasing year by year, and the type of introversion is more common. The etiology and pathogenesis of knee osteoarthritis (KOA) are still unknown, so the early diagnosis and treatment of the disease is very important. Detailed physical examination and perfect imaging examination provide the theoretical basis for the early diagnosis of KOA and prompt the early diagnosis and treatment of the disease. Due to the different level of development of various disciplines and the confusion of people's understanding of the severity of KOA, the contradiction in the treatment of the disease has gradually changed from delayed treatment to over-diagnosis and treatment. According to the different stages of the disease progress, how to formulate a set of targeted, effective treatment plan, to avoid delayed treatment and the occurrence of excessive diagnosis and treatment, has become a new topic. In this paper, the common clinical diagnosis and treatment schemes are introduced one by one, and the indications, advantages and disadvantages are described, and the advantages of each scheme in the treatment of different stages of KOA are emphasized.
