【摘要】：目的對(duì)比觀察半髖關(guān)節(jié)置換術(shù)與內(nèi)固定術(shù)治療股骨轉(zhuǎn)子間骨折的效果。方法選擇股骨轉(zhuǎn)子間骨折患者151例,隨機(jī)分為A組(51例)、B組(48例)、C組(52例),分別采用半髖關(guān)節(jié)置換術(shù)、股骨近端髓內(nèi)釘固定術(shù)、動(dòng)力髖螺釘固定術(shù)進(jìn)行治療。記錄三組手術(shù)時(shí)間、出血量、下床時(shí)間、住院時(shí)間;術(shù)后12個(gè)月采用Harris評(píng)分評(píng)價(jià)髖關(guān)節(jié)功能;記錄術(shù)后12個(gè)月內(nèi)并發(fā)癥及死亡情況。結(jié)果手術(shù)時(shí)間、術(shù)中出血量、下床時(shí)間A組B組C組,兩兩比較差異有統(tǒng)計(jì)學(xué)意義(P均0.05);三組住院時(shí)間比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。A組優(yōu)良率高于B、C組(P均0.05),B、C組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。A、B組并發(fā)癥發(fā)生率低于C組(P0.05),A、B組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。A組術(shù)后12個(gè)月病死率低于B、C組(P0.05),B、C組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論與內(nèi)固定術(shù)治療比較,半髖關(guān)節(jié)置換術(shù)治療股骨轉(zhuǎn)子間骨折效果更佳,病死率及并發(fā)癥發(fā)生率更低。
[Abstract]:Objective to compare the effect of half hip arthroplasty and internal fixation in the treatment of intertrochanteric fracture. Methods A total of 51 patients with intertrochanteric fracture were randomly divided into group A (51 patients with), B and 48 patients with), C, 52 patients). The patients were treated with half hip arthroplasty, proximal femoral intramedullary nail fixation and dynamic hip screw fixation respectively. The time of operation, the amount of bleeding, the time of getting out of bed and the length of hospitalization were recorded, the hip function was evaluated by Harris score at 12 months after operation, and the complications and death were recorded within 12 months after operation. Results the time of operation, the amount of bleeding during operation and the time of getting out of bed in group A, group B and group C were significantly different (P < 0.05), and there was significant difference between the two groups (P < 0.01). There was no significant difference in hospitalization time among the three groups (P0.05,). A group was higher than B, C group (all P 0.05), B, C group had no significant difference (P0.05). A,). The incidence of complications in group B was lower than that in group C (P0.05). There was no significant difference in mortality between group A and group B (P0.05). A group was lower than that in group B and group C at 12 months after operation) (P0.05), B,). There was no significant difference in group C (P 0.05). Conclusion compared with internal fixation, half hip arthroplasty is more effective in the treatment of femoral intertrochanteric fractures, with lower mortality and lower incidence of complications.
