【摘要】：目的:設(shè)計“瘺管剝除括約肌掛浮線引流術(shù)”治療高位肛瘺,并與傳統(tǒng)的“瘺管切開掛線引流術(shù)”治療高位肛瘺進(jìn)行對照,探討瘺管剝除括約肌掛浮線引流術(shù)治療高位肛瘺的臨床療效和安全性。方法:納入符合診斷標(biāo)準(zhǔn)的高位肛瘺病例40例,隨機(jī)分成治療組和對照組,其中瘺管剝除括約肌掛浮線引流術(shù)組(治療組)20例,瘺管切開掛線引流術(shù)組(對照組)20例。觀察兩組在治愈率、復(fù)發(fā)率、愈合時間、肛門功能、肛管動力學(xué)、術(shù)后并發(fā)癥及后遺癥方面的差異。結(jié)果:兩組在創(chuàng)面治愈率、尿潴留方面差異無統(tǒng)計學(xué)意義(P0.05)。兩組在術(shù)后第3、7天疼痛情況有差異(P0.05),第14天疼痛情況無差異(P0.05),治療組疼痛情況較輕(以術(shù)后第3、7天為明顯)。兩組在愈合時間、肛管動力學(xué)、肛門功能方面差異有統(tǒng)計學(xué)意義(P0.05),治療組優(yōu)于對照組。治療組和對照組均臨床痊愈,隨訪半年兩組均無復(fù)發(fā)。結(jié)論:瘺管剝除括約肌掛浮線引流術(shù)治療高位肛瘺與瘺管切開掛線引流術(shù)相比,治愈率相當(dāng),但術(shù)后疼痛輕,愈合時間縮短,能很好的維持肛門正常形態(tài)、保護(hù)肛門功能。
[Abstract]:Objective: to design the "Fistula exfoliation sphincter floating line drainage" for the treatment of high anal fistula, and to compare it with the traditional "fistula incision and drainage" in the treatment of high anal fistula. To evaluate the clinical efficacy and safety of sphincter drainage in the treatment of high anal fistula. Methods: 40 cases of high anal fistula were randomly divided into treatment group (n = 20) and control group (n = 20). There were 20 cases in group A (n = 20) and group B (n = 20) in which sphincter was removed from sphincter. The differences of cure rate, recurrence rate, healing time, anal function, anal dynamics, postoperative complications and sequelae between the two groups were observed. Results: there was no significant difference in wound cure rate and urine retention between the two groups (P0.05). The pain of the two groups was different on the 3rd and 7th day after operation (P0.05), and there was no difference on the 14th day (P0.05). The pain in the treatment group was mild (especially on the 3rd and 7th day after operation). There were significant differences in healing time, anal dynamics and anal function between the two groups (P0.05), and the treatment group was superior to the control group. Both the treatment group and the control group were cured and there was no recurrence in the two groups after half a year of follow-up. Conclusion: the curative rate of fistula exfoliation sphincter floating line drainage is similar to that of fistula incision and thread drainage, but the pain after operation is light, the healing time is shortened, and it can maintain the normal shape of anus and protect anal function.
