【摘要】：目的探討切開復(fù)位內(nèi)固定術(shù)治療跖骨基底部粉碎性損傷(Lisfranc損傷)的臨床療效。方法選取2012年3月至2014年7月復(fù)旦大學(xué)附屬中山醫(yī)院青浦分院骨科住院治療的65例Lisfranc損傷患者作為研究對象,按照手術(shù)方式的不同分為觀察組和對照組,其中觀察組患者36例接受切開復(fù)位內(nèi)固定術(shù)治療,對照組患者29例接受關(guān)節(jié)融合術(shù)治療。比較兩組患者的手術(shù)時間、術(shù)中出血量、術(shù)后住院時間、AOFAS足評分以及術(shù)后并發(fā)癥發(fā)生情況。結(jié)果觀察組患者術(shù)中出血量低于對照組,手術(shù)時間及術(shù)后住院時間高于對照組,差異均有統(tǒng)計學(xué)意義(t=-4.689、4.522、3.120,P均0.05);觀察組術(shù)后的疼痛值得分、功能值得分、力線值得分、總得分以及AOFAS足評分等級均優(yōu)于對照組,差異均有統(tǒng)計學(xué)意義(t=0.547、9.768、12.323、4.745、12.253,P均0.05);觀察組患者并發(fā)癥發(fā)生率為22.22%,對照組為24.14%,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論切開復(fù)位內(nèi)固定對Lisfranc損傷患者具有較明顯療效,值得臨床推廣應(yīng)用。
[Abstract]:Objective to investigate the clinical effect of open reduction and internal fixation in the treatment of comminuted injury of metatarsal base (Lisfranc). Methods from March 2012 to July 2014, 65 patients with Lisfranc injury in the Department of Orthopaedics, Qingpu Branch Hospital, affiliated Hospital of Fudan University were selected as the subjects, and were divided into observation group and control group according to different surgical methods. 36 patients in the observation group were treated with open reduction and internal fixation, and 29 patients in the control group were treated with arthrodesis. The operative time, intraoperative bleeding, postoperative hospital stay, AOFAS foot score and postoperative complications were compared between the two groups. Results the amount of bleeding during operation in the observation group was lower than that in the control group, and the operative time and hospitalization time in the observation group were significantly higher than those in the control group (t = 4.689,4.522,3.120, P < 0.05). The postoperative pain score, functional score, force line score, total score and AOFAS foot score in the observation group were significantly better than those in the control group (t = 0.547, 9.768, 12.323, 4.745, 12.253, P 0.05). The incidence of complications was 22.22% in the observation group and 24.14% in the control group. There was no significant difference between the two groups (P0.05). Conclusion Open reduction and internal fixation is effective in the treatment of Lisfranc injury, and it is worthy of clinical application.
