巨大左心室瓣膜置換術(shù)的臨床價(jià)值及其對心功能的影響

【摘要】:目的探討瓣膜置換術(shù)在巨大左心室患者臨床治療中的應(yīng)用價(jià)值及其對心功能的影響。方法選取2003年8月-2013年10月該院收治的37例患者。所有患者行瓣膜置換術(shù)進(jìn)行治療。分別對患者治療前后不同時(shí)間超聲心動(dòng)圖指標(biāo)、心功能(NYHA)分級(jí)情況進(jìn)行比較,并統(tǒng)計(jì)患者并發(fā)癥及死亡情況。結(jié)果與治療前相比,患者治療后1個(gè)月左心室舒張末期內(nèi)徑(LVEDD)和左心室收縮末內(nèi)徑(LVESD)下降,治療后6個(gè)月LVEDD、LVESD及每博指數(shù)(SVI)下降,治療后1年LVEDD、LVESD及SVI下降,左心室射血分?jǐn)?shù)(LVEF)和左心室短軸縮短率(LVFS)提高。與治療前相比,患者治療后6個(gè)月和1年心功能Ⅰ、Ⅱ級(jí)比率提高,Ⅲ、Ⅳ級(jí)比率下降;颊咝g(shù)后早期死亡2例,死亡率為5.41%(2/37)。室性心律失常15例,低心排出量綜合征14例,肺部并發(fā)癥6例。結(jié)論瓣膜置換術(shù)能夠有效改善巨大左心室患者心功能,且近期術(shù)后并發(fā)癥和死亡率較低,其遠(yuǎn)期療效則需做進(jìn)一步的臨床隨訪研究。
[Abstract]:Objective to evaluate the clinical value of valve replacement in the treatment of giant left ventricle and its effect on cardiac function. Methods 37 patients from Aug. 2003 to Oct. 2013 in our hospital were selected. All patients were treated with valve replacement. The echocardiographic parameters and cardiac function (NYHA) grading were compared before and after treatment, and the complications and death of the patients were counted.
