【摘要】：目的探討關(guān)節(jié)鏡清理聯(lián)合髕股周圍去神經(jīng)化手術(shù)治療髕股關(guān)節(jié)炎的臨床療效。方法選取2014年1月至2015年9月該院收治的髕股關(guān)節(jié)炎膝前疼痛患者100例,采用隨機數(shù)字表法將患者分為觀察組和對照組,每組各50例。對照組給予關(guān)節(jié)鏡清理術(shù)治療,觀察組給予關(guān)節(jié)鏡清理術(shù)聯(lián)合髕股周圍去神經(jīng)化術(shù)治療。比較兩組患者治療前、后的視覺模擬評分(VAS)及治療后的Lysholm和Kujala評分。結(jié)果治療前兩組患者的VAS比較差異無統(tǒng)計學(xué)意義(P0.05)。治療后兩組患者的VAS均顯著降低,且觀察組患者的VAS低于對照組(P0.05)。治療后觀察組、對照組患者的Lysholm評分總分分別為(80.93±4.21)、(74.29±4.48)分,觀察組患者跛行、關(guān)節(jié)絞索、承受重量、疼痛、不穩(wěn)定、上下樓、關(guān)節(jié)腫痛、下蹲起立的膝關(guān)節(jié)Lysholm評分均明顯優(yōu)于對照組(P0.05);治療后觀察組、對照組患者的Kujala評分總分分別為(76.48±6.54)、(68.34±6.22)分,觀察組患者跛行、步行距離、承受重量、下蹲、上下樓、跳躍、跑步、膝關(guān)節(jié)疼痛、長時間屈膝、髕股異�；顒�、膝前腫脹、膝關(guān)節(jié)曲彎受限、大腿肌肉萎縮的髕骨Kujala評分均明顯優(yōu)于對照組(P0.05)。結(jié)論關(guān)節(jié)鏡清理聯(lián)合髕股周圍去神經(jīng)化手術(shù)治療髕股關(guān)節(jié)骨關(guān)節(jié)炎能夠明顯緩解疼痛,改善膝關(guān)節(jié)功能和髕股關(guān)節(jié)功能。
[Abstract]:Objective to investigate the clinical effect of arthroscopic cleaning combined with peripatellofemoral neurosurgery in the treatment of patellofemoral arthritis. Methods from January 2014 to September 2015, 100 patients with anterior knee pain of patellofemoral arthritis were randomly divided into observation group (n = 50) and control group (n = 50). The patients in the control group were treated with arthroscopic cleaning and the observation group were treated with arthroscopic denervation combined with peri-patellofemoral neurolysis. Visual analogue scores (VAS) before and after treatment and Lysholm and Kujala scores after treatment were compared between the two groups. Results there was no significant difference in VAS between the two groups before treatment (P0.05). After treatment, the VAS of the two groups decreased significantly, and the VAS of the observation group was lower than that of the control group (P0.05). After treatment, the total score of Lysholm in observation group and control group was (80.93 鹵4.21), (74.29 鹵4.48 respectively. The patients in observation group had claudication, joint noose, weight bearing, pain, instability, upper and lower stairs, joint swelling and pain. The knee Lysholm scores of squatting and standing knee joints were significantly better than those of the control group (P0.05). After treatment, the total score of Kujala in observation group and control group was (76.48 鹵6.54), (68.34 鹵6.22). The patients in observation group had claudication, walking distance, weight bearing, squat, downstairs, jumping, running and knee joint pain. The Kujala scores of knee flexion, abnormal patellofemoral activity, anterior knee swelling, limited bending of knee joint and atrophy of thigh muscle were significantly better than those in control group (P0.05). Conclusion arthroscopic cleaning combined with peripatellofemoral demineralization can relieve pain and improve the function of knee joint and patellofemoral joint in patients with patellofemoral osteoarthritis.
