【摘要】：目的:評(píng)價(jià)關(guān)節(jié)鏡下肩峰成形術(shù)在肩袖損傷修復(fù)中的臨床療效。方法:2012年5至2014年5月接受關(guān)節(jié)鏡下肩袖損傷縫線橋固定的患者65例,男42例,女23例,隨機(jī)分為兩組,一組術(shù)中同時(shí)行關(guān)節(jié)鏡下肩峰成形手術(shù)(實(shí)驗(yàn)組),另一組術(shù)中行關(guān)節(jié)鏡下肩峰下清理,不做骨質(zhì)的切除(對(duì)照組)。兩組患者年齡、性別、側(cè)別的差異均無統(tǒng)計(jì)學(xué)意義。記錄兩組手術(shù)時(shí)間,比較術(shù)后3個(gè)月、12個(gè)月時(shí)肩關(guān)節(jié)活動(dòng)度、美國加州大學(xué)洛杉磯分校評(píng)分系統(tǒng)(ULCA)、美國肩肘外科醫(yī)師評(píng)分系統(tǒng)(ASES)、視覺模擬評(píng)分(VAS)和Constant評(píng)分變化。結(jié)果:實(shí)驗(yàn)組與對(duì)照組在手術(shù)時(shí)間上兩組差異有統(tǒng)計(jì)學(xué)意義(t=-18.5,P0.05);實(shí)驗(yàn)組與對(duì)照組術(shù)后3個(gè)月、12個(gè)月肩關(guān)節(jié)活動(dòng)范圍、肩關(guān)節(jié)功能評(píng)分方面兩組差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:關(guān)節(jié)鏡下肩袖損傷縫線橋固定術(shù)中進(jìn)行肩峰成形手術(shù)在術(shù)后肩關(guān)節(jié)活動(dòng)度、肩關(guān)節(jié)評(píng)分的改善方面與對(duì)照組無顯著差異,但手術(shù)時(shí)間更長(zhǎng)。
[Abstract]:Objective: to evaluate the clinical effect of arthroscopic acromioplasty in the repair of rotator cuff injury. Methods: from May 2012 to May 2014, 65 patients (42 males and 23 females) with arthroscopic suture bridge fixation of rotator cuff injury were randomly divided into two groups: one group underwent arthroscopic acromioplasty simultaneously (experimental group). Another group underwent arthroscopic subacromial debridement without bone resection (control group). There was no significant difference in age, sex and side difference between the two groups. The operation time of the two groups was recorded, and the range of shoulder motion at 3 and 12 months postoperatively was compared. The UCLA scoring system (ULCA), the American shoulder and elbow surgeon scale system, (ASES), Visual analogue score: (VAS) and Constant. Results: there was significant difference in the operation time between the experimental group and the control group (t = -18.5). There was no significant difference between the experimental group and the control group in the range of shoulder motion and the shoulder function score 3 months and 12 months after operation (P0.05). Conclusion: there is no significant difference in the range of shoulder motion and the improvement of shoulder joint score between arthroscope and the control group, but the operative time is longer.
