定量超聲測(cè)定跟骨骨密度對(duì)骨質(zhì)疏松性骨折的判斷價(jià)值及其與血清骨代謝、骨轉(zhuǎn)化指標(biāo)的相關(guān)性

【摘要】:目的分析定量超聲測(cè)定跟骨骨密度(BMD)對(duì)骨質(zhì)疏松性骨折的判斷價(jià)值,并探討B(tài)MD與患者血清骨代謝、骨轉(zhuǎn)化指標(biāo)的相關(guān)性。方法將2013年8月至2015年8月期間在北京大學(xué)深圳醫(yī)院接受檢查治療的骨質(zhì)疏松性骨折患者72例作為觀察組,另取同期在本院接受健康體檢、不伴骨質(zhì)疏松的老年人群68例作為對(duì)照組,比較兩組患者的定量超聲下BMD值、血清骨代謝、骨轉(zhuǎn)化指標(biāo)值的差異,并進(jìn)一步分析BMD與骨質(zhì)疏松性骨折患者病情的關(guān)系。結(jié)果觀察組患者的BMD為(0.23±0.04)g/cm2、25羥維生素D(25OHD)為(39.27±4.51)nmol/L、骨鈣素N端中分子片段(N-MID)為(56.18±7.29)ng/m L、人降鈣素(HCT)為(2.41±0.32)ng/L,均相應(yīng)低于對(duì)照組的(0.35±0.05)g/cm2、(57.63±6.92)nmol/L、(82.55±9.04)ng/m L、(3.17±0.42)ng/L,而堿性磷酸酶(ALP)為(56.27±6.19)IU/L、Ⅰ型前膠原N末端前肽(PINP)為(42.18±5.29)μg/L、抗酒石酸酸性磷酸酶(TRAP)為(4.63±0.53)U/L、β-膠原降解產(chǎn)物(β-CTX)為(64.39±7.11)ng/m L,均高于對(duì)照組的(27.45±3.38)IU/L、(24.76±3.08)μg/L、(3.11±0.37)U/L、(45.28±5.29)ng/m L,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。經(jīng)一元線性回歸分析發(fā)現(xiàn),骨質(zhì)疏松性骨折患者的BMD值與血清25OHD、N-MID、HCT值呈正相關(guān)(r=0.627、0.701、0.598),與ALP、PINP、TRAP、β-CTX呈負(fù)相關(guān)(r=-0.582、-0.672、-0.693,P0.05)。結(jié)論定量超聲測(cè)定跟骨骨密度可以客觀判斷骨質(zhì)疏松性骨折的嚴(yán)重度,其與患者的血清學(xué)指標(biāo)存在良好的相關(guān)關(guān)系,可以作為臨床病情監(jiān)測(cè)及預(yù)后判斷的可靠手段。
