【摘要】：研究目的:本研究旨在探討血府逐瘀膠囊對(duì)膝下動(dòng)脈PTA術(shù)后療效的影響。研究對(duì)象:選取2015年02月至2016年05月在中國(guó)中醫(yī)科學(xué)院西苑醫(yī)院周圍血管病科住院,符合下肢動(dòng)脈硬化閉塞癥診斷,中醫(yī)辨證存在血瘀證,而且在我科成功接受了膝下動(dòng)脈PTA治療的40例患者。研究方法:將入組的40例患者隨機(jī)分入西藥常規(guī)治療組(對(duì)照組)和西藥常規(guī)治療+中藥治療組(治療組),各20例。對(duì)照組口服阿司匹林100mg,qd;治療組口服阿司匹林100 mg,qd+血府逐瘀膠囊6粒,bid(從術(shù)后第3天開始至術(shù)后半年隨訪終點(diǎn)停用)。在術(shù)前、術(shù)后3天及術(shù)后半年對(duì)患者進(jìn)行相關(guān)實(shí)驗(yàn)室檢查指標(biāo)的檢測(cè)、ABI測(cè)量、Rutherford分級(jí)的記錄。隨訪結(jié)束,對(duì)兩組患者這些指標(biāo)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,評(píng)價(jià)血府逐瘀膠囊對(duì)膝下動(dòng)脈PTA后療效的影響。研究結(jié)果:1.兩組患者在年齡、性別、病史、PTA治療等方面情況基本相似,差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),具有可比性。2.兩組患者的空腹血糖(FBG)、糖化血紅蛋白(HbAlc)、甘油三酯(TG)、低密度脂蛋白(LDL)、白介素-6(IL-6)、超敏C反應(yīng)蛋白(hs-CRP)、纖維蛋白原(Fg)實(shí)驗(yàn)室檢查指標(biāo)在中藥治療前,統(tǒng)計(jì)學(xué)上無(wú)顯著差異(P0.05);術(shù)后半年,治療組較對(duì)照組各項(xiàng)指標(biāo)均有明顯改善,組間相比差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。3.膝下PTA術(shù)后3天,評(píng)估治療組與對(duì)照組之間Rutherford分級(jí)構(gòu)成無(wú)顯著性差異(P0.05)。術(shù)后半年,治療組與對(duì)照組患者Rutherford分級(jí)組間存在差異,有統(tǒng)計(jì)學(xué)意義(P0.05)。4.膝下動(dòng)脈PTA后第3天,測(cè)量?jī)山M患者ABI較術(shù)前均明顯提高,但兩組間無(wú)顯著差異(P0.05);術(shù)后半年,治療組ABI數(shù)值與對(duì)照組ABI數(shù)值有存在差異,有統(tǒng)計(jì)學(xué)意義(P0.05)。隨訪期間無(wú)不良事件發(fā)生。結(jié)論:血府逐瘀膠囊通過其抗動(dòng)脈粥樣硬化、擴(kuò)張血管、改善血液流變性和微循環(huán)、降血脂等作用對(duì)膝下動(dòng)脈PTA術(shù)后療效的維持和改善有一定的干預(yù)作用,對(duì)患者的臨床癥狀、體征及生活質(zhì)量提高有積極的意義。
[Abstract]:Objective: to investigate the effect of Xuefu Zhuyu capsule on PTA of inferior genu artery. Participants: selected from February 2015 to May 2016 in the Department of Peripheral Angiology, Xiyuan Hospital, Chinese Academy of traditional Chinese Medicine, in accordance with the diagnosis of lower extremity arteriosclerosis obliteration, TCM syndrome differentiation exists blood stasis, And 40 patients who were successfully treated with PTA of inferior genu artery in our department. Methods: 40 patients were randomly divided into two groups: the control group (n = 20) and the traditional Chinese medicine group (n = 20). The control group took aspirin 100mg / g QD, and the treatment group took aspirin 100 mg,qd Xuefu Zhuyu capsule (6, bid () from the 3rd day after operation to the end of half a year after operation. The relevant laboratory tests, ABI measurements, and Rutherford grading were performed before, 3 days and 6 months after operation. To evaluate the effect of Xuefu Zhuyu capsule on PTA of inferior genu artery. Results: 1. Two groups of patients in age, sex, medical history, PTA treatment and other aspects of the situation is basically similar, the difference is not statistically significant (P0.05), comparable. 2. Fasting blood glucose (FBG), glycosylated hemoglobin (HbAlc), triglyceride (TG), low density lipoprotein (LDL), interleukin-6 (IL-6), hypersensitive C-reactive protein (hs-CRP), There was no significant difference in fibrinogen (Fg) laboratory index before treatment with Chinese medicine (P0.05). Half a year after operation, the treatment group compared with the control group were significantly improved, the difference between the two groups was statistically significant (P0.05). There was no significant difference in the composition of Rutherford grade between the treatment group and the control group 3 days after PTA (P0.05). Half a year after operation, the treatment group and the control group between the Rutherford grading group differences, there was statistical significance (P0.05). On the 3rd day after PTA of inferior genu artery, the ABI of the two groups was significantly higher than that of the pre-operation, but there was no significant difference between the two groups (P0.05). Half a year after the operation, the ABI value of the treatment group and the control group were significantly different (P0.05). No adverse events occurred during follow-up. Conclusion: Xuefu Zhuyu capsule has some interventional effects on the maintenance and improvement of the curative effect of inferior genu artery PTA through its effects of anti-atherosclerosis, dilating blood vessels, improving blood rheology and microcirculation and lowering blood lipid. The improvement of physical signs and quality of life is of positive significance.
