【摘要】：目的探討雙鋼板內(nèi)固定治療肱骨遠(yuǎn)端粉碎性骨折臨床療效。方法回顧性分析自2010年5月-2014年10月在我院采用雙鋼板固定治療肱骨遠(yuǎn)端粉碎性骨折患者33例,3例失訪,有效例數(shù)為30例,30例成功收集全部患者臨床資料,包括手術(shù)資料與術(shù)后療效。其中男性18例,女性12例,年齡范圍20-60歲,平均年齡36歲。按照AO/ASIF分型：C1型：12例,C2型10例,C3型8例,4例術(shù)前有尺神經(jīng)損害,1例術(shù)前有橈神經(jīng)損傷。均有外傷史,其中摔傷7例,高處墜落傷13例,車禍傷10例。受傷到手術(shù)時(shí)間3-8d,平均5d。通過對(duì)患者的肘、腕關(guān)節(jié)的疼痛、運(yùn)動(dòng)、X線表現(xiàn)以及肘關(guān)節(jié)功能評(píng)分進(jìn)行研究。結(jié)果：隨訪時(shí)間6-24個(gè)月,平均18個(gè)月。根據(jù)Morrey肘關(guān)節(jié)功能評(píng)價(jià)標(biāo)準(zhǔn)對(duì)術(shù)后患者的肘關(guān)節(jié)功能進(jìn)行評(píng),療效優(yōu)14例(95-100分),良好11例(80-94分),中4例(50-79分),差1例(50分),總優(yōu)良率為83.4%。結(jié)論：本組30例均為肱骨遠(yuǎn)端粉碎性骨折,通過雙鋼板內(nèi)固定治療,術(shù)后隨訪臨床可獲得較滿意的肘關(guān)節(jié)活動(dòng)度,無骨折不愈合,異位骨化1列,尺神經(jīng)損傷1例,雙鋼板內(nèi)固定治療肱骨遠(yuǎn)端粉碎性骨折是一種較好的臨床方法,具有生物力學(xué)的優(yōu)勢(shì),固定牢靠療效確切,并發(fā)癥少,臨床療效滿意。
[Abstract]:Objective to investigate the clinical effect of double plate fixation for comminuted distal humeral fractures. Methods from May 2010 to October 2014, 33 cases of comminuted fractures of distal humerus were treated with double plate fixation in our hospital. Including surgical data and postoperative results. There were 18 males and 12 females, aged 20-60 years, with an average age of 36 years. According to AO/ASIF classification: C1 type: 12 cases, C2 type 10 cases, C3 type 8 cases, 4 cases had ulnar nerve damage before operation, 1 case had radial nerve injury before operation. There were 7 cases of fall injury, 13 cases of falling injury and 10 cases of traffic accident injury. The time from injury to surgery was 3-8 days, with an average of 5 days. The pain, movement, X-ray findings and functional score of the elbow and wrist were studied. Results: the follow-up time was 6-24 months (mean 18 months). According to Morrey's elbow function evaluation standard, 14 cases (95-100 points) had excellent results, 11 cases (80-94 points) were good, 4 cases (50-79 points), 1 case (50%) poor, 4 cases (50-79 points), 1 case (50%) poor. The total excellent and good rate was 83.4%. Conclusion: all 30 cases of comminuted fractures of distal humerus were treated with double plate internal fixation. Satisfactory elbow motion, no fracture nonunion, ectopic ossification 1 column, ulnar nerve injury 1 case were obtained after operation. The treatment of comminuted fracture of distal humerus with double plate internal fixation is a good clinical method with the advantages of biomechanics, firm fixation, few complications and satisfactory clinical effect.
