【摘要】：目的：探討通過雙Endo-Button固定雙束解剖重建內(nèi)側(cè)髕股韌帶治療復(fù)發(fā)性髕骨脫位的短期療效。 方法：自2012年9月至2015年3月收治12例(12膝)復(fù)發(fā)性髕骨脫位患者。其中女性10例，男性2例；年齡19-48歲，平均27歲。其中10膝使用自體半腱肌，2膝使用同種異體肌腱。所有患者均行髕外側(cè)支持帶松解術(shù)，手術(shù)使用雙束移植肌腱，于髕骨內(nèi)側(cè)緣建立雙骨隧道，以模擬MPFL髕側(cè)止點，股骨止點處建立單骨隧道。本術(shù)式的關(guān)鍵是使用雙束肌腱通過骨隧道和雙Endo-button固定進(jìn)行MPFL解剖重建。術(shù)后所有病例均獲得隨訪，隨訪時間為6至24個月，平均14個月；臨床療效評估包括膝關(guān)節(jié)Lysholm評分、Kujala評分及Tegner評分。 結(jié)果：所有患者術(shù)后恐懼試驗均陰性，隨訪期間無髕骨骨折及再脫位發(fā)生。Lysholm評分(94.67±2.93)分、Kujala評分(94.33±2.74)及Tegner評分(5.83±0.72)分，與術(shù)前比較差異有統(tǒng)計學(xué)意義。 結(jié)論：通過雙Endo-button固定雙束解剖重建內(nèi)側(cè)髕股韌帶重建治療復(fù)發(fā)性髕骨脫位主觀癥狀改善及膝關(guān)節(jié)功能恢復(fù)滿意且無并發(fā)癥發(fā)生，，近期療效可靠(P＜0.05)。
[Abstract]:Objective: to investigate the short-term effect of reconstruction of medial patellofemoral ligament by double Endo-Button fixation and double bundle anatomy in the treatment of recurrent patellar dislocation. Methods: from September 2012 to March 2015, 12 patients (12 knees) with recurrent patellar dislocation were treated. There were 10 females and 2 males, aged 19-48 years, with an average age of 27 years. Autogenous semitendinosus was used in 10 knees and allogeneic tendon in 2 knees. All patients were treated with lateral patellar retinaculum release. Double bundle tendon graft was used to establish double bone tunnel at the medial edge of patella in order to simulate MPFL patellar lateral insertion point and femur end point to establish single bone tunnel. The key to this procedure is to use double bundle tendon through bone tunnel and double Endo-button fixation for MPFL anatomical reconstruction. All the patients were followed up for 6 to 24 months with an average of 14 months. The evaluation of clinical efficacy included knee joint Lysholm score, Kujala score and Tegner score. Results: all the patients were negative in fear test after operation, and no patellar fracture or redislocation occurred during follow-up. Lysholm score (94.67 鹵2.93), Kujala score (94.33 鹵2.74) and Tegner score (5.83 鹵0.72). The difference was statistically significant compared with that before operation. Conclusion: reconstruction of medial patellofemoral ligament by double Endo-button fixation and reconstruction of medial patellofemoral ligament can improve the subjective symptoms of recurrent patellar dislocation and restore the function of knee joint with no complication. The short-term curative effect is reliable (P < 0. 05).
