【摘要】：背景:微創(chuàng)經(jīng)椎間孔椎間融合治療腰椎滑脫具有組織損傷小、術(shù)中出血少、術(shù)后住院時(shí)間短等優(yōu)勢,但其與開放經(jīng)椎間孔椎體間融合治療腰椎滑脫的療效比較沒有經(jīng)過嚴(yán)格的循證醫(yī)學(xué)檢驗(yàn)。目的:系統(tǒng)性評價(jià)微創(chuàng)經(jīng)椎間孔椎體間融合和開放經(jīng)椎間孔椎體間融合治療腰椎滑脫的臨床療效及安全性。方法:計(jì)算機(jī)檢索萬方數(shù)據(jù)庫、CNKI、PubM ed、Cochrane Library等數(shù)據(jù)庫,以腰椎滑脫、微創(chuàng)、開放、經(jīng)椎間孔椎體間融合術(shù)、minimally invasive、open transforaminal lumbar interbody fusion、微創(chuàng)經(jīng)椎間孔椎體間融合、開放經(jīng)椎間孔椎體間融合等為檢索詞。由2名作者獨(dú)立對納入的文獻(xiàn)進(jìn)行質(zhì)量評價(jià)及數(shù)據(jù)提取,用Rev Man 5.3軟件進(jìn)行Meta分析。結(jié)果與結(jié)論:共納入10篇文獻(xiàn),其中回顧性研究7篇,隨機(jī)對照研究3篇,共納入病例963例,微創(chuàng)組489例,開放組474例。(1)Meta分析結(jié)果顯示:在手術(shù)時(shí)間、術(shù)后并發(fā)癥發(fā)生率、末次隨訪融合率等方面2組差異無顯著性意義,說明2種術(shù)式均能明顯改善患者疼痛及功能障礙;(2)在術(shù)中出血量、術(shù)中射線暴露時(shí)間等方面差異有顯著性意義;(3)結(jié)果說明,微創(chuàng)經(jīng)椎間孔椎體間融合創(chuàng)傷小、術(shù)中及術(shù)后出血少,與開放經(jīng)椎間孔椎體間融合相比具有相似的術(shù)后功能改善情況,且不增加手術(shù)時(shí)間和并發(fā)癥的發(fā)生率,是一種安全、有效的手術(shù)方式。
[Abstract]:Background: minimally invasive transforaminal fusion for lumbar spondylolisthesis has the advantages of less tissue damage, less intraoperative bleeding, and shorter hospital stay after operation. However, the efficacy of open transforaminal interbody fusion in the treatment of lumbar spondylolisthesis has not been strictly tested by evidence-based medicine. Objective: to evaluate the clinical efficacy and safety of minimally invasive transforaminal fusion and open transforaminal interbody fusion in the treatment of lumbar spondylolisthesis. Methods: the database of Wanfang, CNKI,PubM ed,Cochrane Library and so on were searched with lumbar spondylolisthesis, minimally invasive, open, interbody fusion through intervertebral foramen and minimally invasive,open transforaminal lumbar interbody fusion, minimally invasive interbody fusion of intervertebral foramen. Open intervertebral fusion through intervertebral foramen is the key word. The quality evaluation and data extraction were carried out by two authors, and Meta analysis was carried out with Rev Man 5.3 software. Results and conclusion: a total of 10 articles were included, including 7 retrospective studies and 3 randomized controlled studies. A total of 963 cases were included, 489 cases in the minimally invasive group and 474 cases in the open group. (1) the results of Meta analysis showed that the time of operation was in the operation time. There was no significant difference in the incidence of postoperative complications and the fusion rate of the last follow-up between the two groups, indicating that the two methods could significantly improve the pain and dysfunction of the patients. (2) there were significant differences in the amount of intraoperative bleeding and the time of radiation exposure during the operation. (3) the results showed that the minimally invasive transforaminal fusion had less trauma, less intraoperative and postoperative bleeding, similar to the open transforaminal interbody fusion, and did not increase the operative time and the incidence of complications. It is a safe and effective method of operation.
