【摘要】：目的研究甲狀旁腺激素聯(lián)合葛根素對老年骨質(zhì)疏松性股骨粗隆骨折術(shù)后的影響,為老年骨質(zhì)疏松性粗隆骨折尋找安全可靠、療效良好的治療方案。方法 2013年1月至2015年1月間收治的老年性骨質(zhì)疏松性股骨粗隆骨折88例,采用隨機數(shù)字表法將其分為治療組和對照組,每組44例,兩組患者均接受PFNA治療。對照組予以葛根素治療,治療組術(shù)后加用甲狀旁腺激素治療,比較兩組患者治療后患者視覺模擬痛疼評分(VAS)、髖關(guān)節(jié)Harris功能評分、骨折愈合時間及股骨頸及腰椎的骨密度改變。結(jié)果術(shù)后1周、3個月、6個月兩組患者VAS評分均明顯降低,且治療組患者評分明顯低于對照組(P0.05);術(shù)后3個月、6個月兩組患者的Harris評分均明顯上升,且治療組患者評分明顯高于對照組(P0.05);治療組骨折愈合的時間明顯少于對照組(P0.05);術(shù)后3個月、6個月兩組患者股骨頸及腰椎的骨密度,治療組患者骨密度較術(shù)前顯著改善(P0.05),而對照組治療前后上述指標(biāo)無明顯變化(P0.05)。結(jié)論甲狀旁腺激素聯(lián)合葛根素可以明顯加速老年骨質(zhì)疏松性粗隆骨折愈合、改善預(yù)后。
[Abstract]:Objective to study the effect of parathyroid hormone (PTH) combined with puerarin on senile osteoporotic femoral trochanteric fracture and to find a safe and effective treatment for senile osteoporotic trochanteric fracture. Methods 88 cases of senile osteoporotic femoral trochanteric fractures were treated from January 2013 to January 2015. They were randomly divided into treatment group (n = 44) and control group (n = 44). The patients in both groups were treated with PFNA. The patients in the control group were treated with puerarin and the patients in the treatment group were treated with parathyroid hormone after operation. The visual analogue pain score (VAS),) and Harris score of hip joint were compared between the two groups after treatment. Fracture healing time and bone mineral density of femoral neck and lumbar vertebrae. Results at 1 week, 3 months and 6 months after operation, the VAS scores of the two groups were significantly lower than those of the control group (P0.05), and the scores of the patients in the treatment group were significantly lower than those in the control group (P0.05). After 3 months, 6 months after operation, the Harris scores of the two groups were significantly increased, and the scores of the patients in the treatment group were significantly higher than those in the control group (P0.05), the time of fracture healing in the treatment group was significantly less than that in the control group (P0.05). The bone mineral density of femoral neck and lumbar vertebrae was significantly improved in the treatment group 3 months and 6 months after operation (P0.05), but there was no significant change in the control group before and after treatment (P0.05). Conclusion parathyroid hormone combined with puerarin can accelerate the healing of osteoporosis fracture and improve the prognosis.
