可吸收形狀記憶聚氨酯腰椎間融合器的生物力學(xué)評價

【摘要】:目的:評價自主研制的可吸收形狀記憶聚氨酯(shape memory polyurethane,SMPU)腰椎間融合器(cage)在尸體腰椎單節(jié)段的生物力學(xué)特性。方法:將15具腰骶椎(L1?S1)標(biāo)本隨機(jī)分為3組:自體髂骨組(n=5),SMPU cage組(n=5),聚醚醚酮(polyetheretherketone,PEEK)cage組(n=5)。通過腰椎側(cè)方入路,摘除L3?4椎間盤髓核后植入3種內(nèi)植物,在力學(xué)試驗(yàn)機(jī)上測試每組標(biāo)本術(shù)前與術(shù)后的運(yùn)動范圍(range of motion,ROM)及軸向破壞載荷。結(jié)果:ROM結(jié)果顯示:術(shù)前自體髂骨組、SMPU cage組及PEEK cage組在前屈、后伸、左側(cè)彎、右側(cè)彎、左旋轉(zhuǎn)、右旋轉(zhuǎn)6個方向上的ROM相比較,差異均無統(tǒng)計學(xué)意義(P0.05)。自體髂骨組在6個方向上的術(shù)后ROM均較術(shù)前明顯增加(P0.05);SMPU cage組在6個方向上的術(shù)后ROM均較術(shù)前明顯減少(P0.05);PEEK cage組在6個方向上的術(shù)后ROM均較術(shù)前明顯減少(P0.05)。SMPU cage組與PEEK cage組術(shù)后在6個方向上的ROM均顯著低于自體髂骨組(P0.05);SMPU cage組與PEEK cage組術(shù)后在6個方向上的ROM相比較,差異無統(tǒng)計學(xué)意義(P0.05)。軸向壓縮測試結(jié)果顯示:PEEK cage組的破壞載荷較SMPU cage組和自體髂骨組明顯增加(P=0.000),SMPU cage組的破壞載荷較自體髂骨組明顯增加(P=0.000)。結(jié)論:形狀記憶聚氨酯椎間融合器能滿足腰椎椎間融合的初始穩(wěn)定性,并能夠承載腰椎的生理負(fù)荷。
[Abstract]:Aim: to evaluate the biomechanical characteristics of an absorbable shape memory polyurethane (shape memory polyurethane,SMPU) lumbar interbody fusion cage (cage) in single segment of cadaveric lumbar spine. Methods: fifteen lumbosacral vertebrae (L1?S1) specimens were randomly divided into 3 groups: autogenous iliac bone group (n = 5) (n = 5), polyether ether ketone (polyetheretherketone,PEEK) group (n = 5) cage group (n = 5). Three kinds of implants were implanted into the lumbar disc nucleus pulposus by lateral approach of lumbar vertebrae. The range of motion (range of motion,ROM) and the axial damage load of each group were measured by mechanical testing machine before and after operation. Results: the results of ROM showed that there was no significant difference in ROM between, SMPU cage group and PEEK cage group in 6 directions: anterior flexion, extension, left bend, right bend, left rotation and right rotation (P0.05). The postoperative ROM in the autogenous iliac bone group was significantly higher than that in the preoperative group (P0.05). The postoperative ROM in the); SMPU cage group was significantly lower than that in the pre-operation group (P0.05). The postoperative ROM of PEEK cage group in 6 directions was significantly lower than that of preoperative group (P0.05). The ROM of). SMPU cage group and PEEK cage group were significantly lower than that of autogenous iliac bone group (P0.05). There was no significant difference in ROM in 6 directions between SMPU cage group and PEEK cage group (P0.05). The results of axial compression test showed that the damage load of: PEEK cage group was significantly higher than that of SMPU cage group and autogenous iliac bone group (P0. 000), SMPU cage group was significantly higher than that of autologous iliac bone group (P0. 000). Conclusion: shape memory polyurethane interbody fusion cage can satisfy the initial stability of lumbar interbody fusion and can bear the physiological load of lumbar vertebrae.
