【摘要】：目的系統(tǒng)評價(jià)髓核摘除結(jié)合棘突間分離系統(tǒng)動(dòng)態(tài)固定治療腰椎間盤突出癥(LDH)的臨床療效。方法計(jì)算機(jī)檢索建庫至2016年7月Cochrane Library、Pub Med、Medline、Academic Search Premier、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫、中國知網(wǎng)、維普和萬方數(shù)據(jù)庫文獻(xiàn),同時(shí)輔助手工檢索,收集采用髓核摘除結(jié)合棘突間分離系統(tǒng)動(dòng)態(tài)固定與單純髓核摘除治療LDH的臨床對照研究(分別為觀察組和對照組)。采用Revman5.3軟件對相關(guān)數(shù)據(jù)進(jìn)行Meta分析。結(jié)果納入11篇文獻(xiàn),共675例LDH患者,其中觀察組312例、對照組363例。Meta分析結(jié)果顯示,兩組手術(shù)時(shí)間、切口長度、術(shù)中出血量比較差異均無統(tǒng)計(jì)學(xué)意義(P均0.05);觀察組術(shù)后1年疼痛視覺模擬評分、Oswestry功能障礙指數(shù)問卷表評分均低于對照組,術(shù)后1年及2年JOA評分、術(shù)后優(yōu)良率、術(shù)后責(zé)任節(jié)段椎間高度均高于對照組(P均0.01)。結(jié)論髓核摘除結(jié)合棘突間分離系統(tǒng)動(dòng)態(tài)固定治療LDH療效好,患者術(shù)后責(zé)任節(jié)段椎間高度維持好、腰背痛殘留少。
[Abstract]:Objective to evaluate the clinical effect of nucleus pulposus removal combined with dynamic fixation of interspinous process in the treatment of lumbar disc herniation (LDH). Methods Cochrane Library,Pub Med,Medline,Academic Search Premier, Chinese Biomedical Literature Database, China knowledge Network, Weip and Wanfang databases were searched by computer until July, 2016. To collect the clinical control study of LDH treated by nucleus pulposus extirpation combined with dynamic fixation of spinous process separation system and simple removal of nucleus pulposus (observation group and control group respectively). The related data were analyzed by Meta using Revman5.3 software. Results A total of 675 patients with LDH were included in 11 articles. There were 312 cases in the observation group and 363 cases in the control group. The results of Meta analysis showed that there was no significant difference between the two groups in the operation time, the length of incision and the amount of intraoperative bleeding (P 0.05). The visual analogue score of pain and the score of Oswestry dysfunction index questionnaire in the observation group were lower than those in the control group one year and two years after operation. The excellent and good rate and the intervertebral height of the responsible segment were higher in the observation group than in the control group (all P 0.01). Conclusion the removal of nucleus pulposus combined with dynamic fixation of spinous process separation system is effective in the treatment of LDH.
